Morphometric studies of afferent arterioles in spontaneously hypertensive rats demonstrated that cilazapril increased luminal diameter and that the antihypertensive effect persisted after withdrawal of the ACE inhibitor, suggesting a pathogenic role for afferent arteriole diameter in the development of high blood pressure in spontaneously hypertensive rats.

The increase in sodium pump density may participate in initiation of cell adaptation of distal segments to chronic increased salt delivery. Inhibition of ACE reverses the torasemide induced increase in apparent sodium pump density in principal cells and decreases hypertrophy.
