Lancet 1997, 350:911-917. The EOI study compared treatment with 18 weeks of DOX/CDP with a 44-week multiagent regimen based on the Memorial Sloan-Kettering T10 protocol. Despite concerns about lack of dose intensity achieved with the longer regimen, comparable histologic responses and overall survival were seen with short intensive therapy.

In this revised meta-analysis of MTX dose intensity for the treatment of osteosarcoma, the authors excluded studies in which postoperative chemotherapy was modified based on histologic response. The results still clearly demonstrate a significant correlation between MTX dose intensity and survival.

This small study of patients with metastatic osteosarcoma suggests that achievement of complete remission using neoadjuvant therapy (MTX, CDP, DOX, and IFOS) followed by simultaneous excision of both the primary tumor and pulmonary metastases results in survival rates comparable to the rates of patients without metastases. Further, there was a significant rate of false-positive diagnostic chest CTs.

Loss of heterozygosity has been identified in a band on chromosome 3q in over two-thirds of patients with osteosarcoma, suggesting the presence of a novel tumor suppressor gene in this region. The LOH frequency approaches that for p53 and RB1, two gene products previously shown to be important in osteosarcoma development.

Using a model of osteosarcoma spontaneous metastasis following orthotopic implantation in the tibia of mice, the authors have demonstrated that vaccination with lethally irradiated osteosarcoma cells expressing the T-cell costimulatory B7-1 molecule significantly decreased metastatic frequency and increased survival. These effects were synergistic with the effects of intraperitoneal MTX.

Artif Organs 1997, 21:413-417. In this paper, the authors describe the first clinical application of an endoprosthesis that can be extended via an externally applied electromagnet. Despite manufacturing problems, this device holds promise for managing the problem of leg length discrepancy in children undergoing surgical excision of long bone tumors.

A cost analysis is presented that suggests that endoprosthetic reconstruction is more cost effective than amputation and external prostheses for femoral tumors. These data combined with previously published quality of life studies support the trend away from amputation for lower extremity bone tumors.

99mTc-MIBI imaging suggests that the latter may have advantages in evaluating osteosarcoma response to therapy. Both radionuclide angiography and scintigraphy can be performed with the latter agent, which may allow a more accurate assessment of response to therapy.
