The results suggest that the progression of cartilage degeneration is influenced by remodeling of subchondral bone.

This article reviews evidence for the role of subchondral bone and calcified cartilage in the initiation and progression of osteoarthrosis. It concludes that increased subchondral density may not be necessary to initiate osteoarthritic changes but may be necessary for progression. It also concludes that only changes in subchondral bone and calcified cartilage close to the joint cartilage are important to the disease process.

It shows that the texture of the trabecular bone surface and the shape of individual trabeculae are different in bone from osteoarthritic subjects compared with those without osteoarthritis.

    • Li B, Aspden RM: Mechanical and material properties of the subchondral bone plate from the femoral head of patients with osteoarthritis or osteoporosis. Ann Rheum Dis 1997, 56:247-254. This article shows not only that the density and stiffness of subchondral bone as a material may decrease in osteoarthritis, but also that the subchondral plate gets thicker. It concludes that changes in subchondral bone alone cannot explain the breakdown of cartilage in osteoarthritis but does not adequately consider the properties of the subchondral plate as a structure.

    • Mansell JP, Tarlton JF, Bailey AJ: Biochemical evidence for altered subchondral bone collagen metabolism in osteoarthritis of the hip. Br J Rheumatol 1997, 36:16-19. This article shows that trabecular bone from subjects with osteoarthritis expresses more matrix metalloproteinase-2 and alkaline phosphatase than osteoporotic or normal bone. It suggests that this facilitates bone remodeling as the joint tries to adapt to the effects of cartilage degeneration or altered mechanical loading.
