A prospective study of 70 consecutive patients with penetrating ocular trauma for whom intraoperative cultures were obtained. Twenty patients (29%) had positive cultures but only nine (13%) were diagnosed with clinical endophthalmitis. Seven (78%) of these nine patients with clinical endophthalmitis had positive cultures. Positive culture results directly influenced management decisions in cases with clinical evident endophthalmitis. However, routine intraoperative bacterial cultures did not help to identify patients in whom endophthalmitis would develop.

Fifteen percent to 20% of Staphylococcus epidermidis strains isolated from human conjunctiva exhibit slime formation and may be pathogens causing inflammation. Topical application, for example, eye drops, of a combination of two antimicrobial agents is usally applied to treat such infections. Any antimicrobial agent having a postantibiotic effect is generally administered in a twice-a-day regimen.

Timing for other scenarios is less clear, however. Mieler and Mittra [22••], after analysis of all available information, generally perform vitrectomy at approximately 7 to 10 days after penetrating ocular trauma.

A swine model of ocular trauma was used to determine the extent of penetration of systematically administered gentamicin and ceftazidime to the vitreous cavity of traumatized and normal eyes. Ceftazidime achieved an intravitreal concentration 33 times the minimum inhibitory concentration of Haemophilus species and twice that of Pseudomonas species in traumatized eyes after systemic administration. Gentamicin did not appear to enter the traumatized eye at appreciable levels after systemic administration.

They employed the Sawchuck-Zaske equation to calculate vancomycin clearance (CL) and volume of distribution (Vd) on the basis of three different plasma concentrations of vancomycin, and they used CL and Vd values as prior information before employing the parameter estimation and dosage adjustment program to treat a patient with postsurgical endophthalmitis caused by meticillin-resistant Staphylococcus aureus.

Risk factors for infection include retained intraocular foreign body, a rural setting, delay in primary wound closure, and disruption of the crystalline lens. Treatment includes intravitreal, periocular, and systemic antibiotics. Recent and past literature supporting these recommendations, as well as the authors specific prevention and treatment protocols for post-traumatic endophthalmitis, are discussed in this review.

The authors developed an in vitro model to evaluate antibacterial effects of antibiotics in infusion solutions. Suspensions of some organisms were incubated for 30, 60, and 120 minutes at room temperature with vancomycin or gentamicin. Most of the organisms were not affected by exposure to the antibiotics for up to 140 minutes. Exposure to antibiotics for a short period of time such as during intraocular surgery generally has no effect on organisms commonly responsible for endophthalmitis.

The authors study the effect of intracameral povidone-iodine 0.001% in reducing bacterial colonization in experimental cataract surgery. In an experimental in vitro model, the anterior chambers were inoculated with Staphylococcus aureus and Pseudomonas aeruginosa prior to surgery. There was a statistically significant (P < 0.05) difference in bacterial colonization in the 24 hours samples between eyes that received intracameral injection of povidone-iodine and eyes that were controls.

In the first postoperative day, povidone-iodine ophthalmic solution prevented increases in conjunctival bacterial colony-forming units and decreased the species compared with cases treated with antibiotic. The authors determined whether these beneficial effects of povidone-iodine could be sustained during the first postoperative week. They concluded that povidone-iodine ophthalmic solution is an alternative to postoperative topical antibiotics because of its effectiveness in controlling conjundival bacterial colony-forming units and species, its relatively low cost, and its availability.

Baseline factors correlating with higher rates of both gram-negative and other gram-positive isolates were corneal infiltrate, cataract wound abnormalities, afferent pupillary defect, loss of red reflex, initial light perception-only vision, and symptom onset within 2 days of surgery. Gram-negative organisms did not grow in any eyes in which a retinal vessel could be visualized, and 61.9% of these eyes had equivocal or no growth. Diabetes mellitus was associated with a higher yield of gram-positive, coagulase-negative microccoci. Eye pain was not a discriminator for culture results.

The authors state that "omission of systemic antibiotics can reduce toxic effects, cost, and length of hospital stay." It must be strongly emphasized that a single injection of intravitreal antibiotics is not uniformly effective in eradicating bacterial endophthalmitis in either laboratory experiments or in patients. Although the best strategy for additional treatment is not known, a second intravitreal antibiotic injection may be valuable and intravenous therapy should also be considered. Intravenous therapy may be particularly useful in eyes with concomitant anterior loci of infection, such as blebitis and deep stich abscesses.

This classification system categorizes ocular injuries at the time of initial examination. It is designed to promote the use of standard terminology and assessment, with applications to clinical management and research studies regarding eye injuries.
