Smeets HJM, Knoers VVAM, van de Heuvel LPWJ, Lemmink HH, Schroder CH, Monnens LAH: Hereditary disorders of the glomerular basement membrane. Pediatr Nephrol 1996, 10:779-788. Increased knowledge of the biochemical composition of the glomerular basement membrane and the introduction of molecular genetics has contributed in the understanding of hereditary disorders of the glomerular basement membrane. In this review, three disorders are highlighted: Alport's syndrome, nail-patella syndrome, and Drash syndrome.

Benign familial hematuria due to mutation of the type IV collagen a4 gene
-
Lemmink HH, Nillesen WN, Mochizuki T, Schroder CH, Brunner HG, van Oost BA, Monnens LAH, Smeets HJM: Benign familial hematuria due to mutation of the type IV collagen a4 gene. J Clin Invest 1996, 98:1114-1118. The authors present the linkage of benign familial hematuria with the COL4A3 and COL4A4 genes at 2q35-37. A glycine to glutamic acid substitution was identified in the collagenous region of the COL4A4 gene. They conclude that type IV collagen defects cause both benign hematuria and Alport's syndrome. They suggest that patients with benign familial hematuria can be carriers of autosomal recessive Alport's syndrome.

Scheinman J: Glycogen storage disease. In Pediatric Nephrology, edn 4. Edited by Holliday M, Barratt TM, Avner ED. Baltimore: Williams 4 Wilkins, 1998, in press. Starting from the biochemical basis of the disease, the consequences of altered pathways and the renal adaptative mechanisms to the metabolic injury, the author emphasizes that knowledge of the clinical features is required to reach safe and optimal management in GSD-I patients.

Transplantation for primary hyperoxaluria in the USA
-
Scheinman JI, Alexander M, Campbell ED, Chan JC, Latla K, Cochat P: Transplantation for primary hyperoxaluria in the USA. Nephrol Dial Transplant 1995, 10:S42-S46. A survey of US data (NAPRTCS and USRDS) was made for transplantation in PH. The results showed that transplant is better than no transplant; cadaver donation results are poor; living related kidney donation can succeed; and kidney-liver transplant is still problematic in the United States. Until a more cooperative effort can be achieved, isolated kidney living related donor transplant is preferable and does not preclude kidney-liver transplant later.
