    • 2 antagonists, proton pump inhibitors, and antacids all at standard doses do not prevent toxic reactions in the gastrointestinal tract. In fact, their use may be associated with more NSAID-induced toxic reactions by perhaps masking symptoms.

    • Prevention of nonsteroidal antiinflammatory drug-induced gastrointestinal mucosal injury: A metaanalysis of randomized controlled clinical trials

This complicated study supports the notion that H. pylori-infected patients should have the infection eradicated with appropriate therapy whether or not they are concomitantly treated with NSAIDs to prevent recurrence of an ulcer. Unfortunately, the investigators did not demonstrate whether or not there was a significant interaction between infection and NSAIDs in inducing an upper gastrointestinal ulcer or its complications.

J Clin Invest 1997, 99:1231-1237. This important paper demonstrates that there may yet be important effects of highly selective COX-2 inhibition that we do not yet appreciate. Whether or not cartilage repair is modulated through upregulation of COX-2 is still debatable, but this study demonstrates that osteoarthritic cartilage expiants in tissue culture have increased levels of prostaglandins in the media due to induction of COX-2 activity. Whether this is caused by the experimental model or a natural event remains unknown.
