Arch Ophthalmol 1996, 114:1189-1192. A multicenter retrospective study evaluating acute retinal arterial occlusion patients who underwent transthoracic echocardiography. Patients were divided into those of high or low cardioembolic risk. High cardioembolic patients were 25 times more likely to receive anticoagulation or cardiac surgery based on their transthoracic echocardiographic result.

Br J Ophthalmol 1998, in press. In this multicenter retrospective study, transthoracic echocardiographic results are analyzed with respect to visible embolic status. Forty-one patients (39%) had evidence of visible retinal emboli. Twelve patients (11%) received anticoagulation or cardiac surgery based on the results of transthoracic echocardiography. Of the 12 patients who had an alteration in systemic management, only six had visible retinal emboli (sensitivity = 50%). The presence of an embolus did not significantly increase the probability for anticoagulation or cardiac surgery.

Can J Ophthalmol (in press). An observer variability study was conducted on 42 observers to determine the degree of agreement regarding the qualitative assessment of visible retinal emboli. Observers viewed 17 fundus photographs of embolic arterial occlusion and were asked to classify emboli categorically as cholesterol, calcific, or other. Both interobserver and intraobserver agreement were poor.
