Ann Neurol 1997; 42:885-890. A new locus at Chr1q21-23 was reported in three French families with hemiplegic migraine based on a 2-point LOD score of 4.21, θ=0.07 among the 24 affected individuals. Two patients had an attack-related seizure and haplotype analysis suggested one phenocopy and five nonpenetrants, including a discordant twin pair. The families were clinically normal otherwise. Four other families did not link to the Chr1q21-23 or Chr19 FHM loci, demonstrating further genetic heterogeneity.

A mildly expanded CAG repeat in the 3 translated end of three of the six isoforms for the brain expressed CACNL14A calcium channel was found to be associated with a late onset, progressive ataxia called SCA6. The SCA6 expansion disorder is allelic with hemiplegic migraine and episodic ataxia-2. All three disorders are associated with different types of mutations in the CACNL1A4 gene.

One SCA6 family with a CAG repeat size of 23 in the Chr19p CACNL1A4 gene presented with intrafamilial variability of phenotype and no other mutations in the coding sequence of the gene. Another SCA6 family was reported to have an episodic ataxia phenotype in a single member with CAG expansion of 20, and a progressive ataxia only was seen in a member with 25 allele expansion. The authors emphasized clinical variability and the potential for phenotypic overlap between SCA6 and EA-2.

A new locus for hemiplegic migraine at Chr1q31 was described in a four-generation family with stereotypic attacks and no cerebellar signs, in a candidate region for a brain expressed calcium channel. The multipoint LOD score was 3.32 near marker D1S249, using a penetrance of 80% and phenocopy rate of 1/50. A single phenocopy and two nonpenetrant family members who carried the disease haplotype were described.

Migraine without aura only was common in many members. Linkage to the Chr19p and benign neonatal convulsions loci on Chr20 and Chr8 were excluded and an affected only analysis found mild evidence for cotransmission of the two disorders.

A large population-based study with case-controls and direct interviews found an overall relative risk of 1.5 for migraine, which is lower than previous reports from similar studies. Relative risk in migraine with aura was only slightly higher in family members of probands with migraine, also in contrast to a previous study. Importantly, disability with migraine in the proband was associated with a statistically significant higher relative risk of 2.17. Gender differences in migraine risk could not be adequately assessed.
