Assessement of a set of medical, sociodemographic, psychological and work-related parameters pre and post-treatment were performed using standardized instruments. A well elaborated statistical analysis. Main independant predictors for recurrence of pain, disability and depression were orthopaedic findings and job satisfaction; the latter being positively correlated with objective type of work.

    • Hildebrandt J, Pfingsten M, Saur P, Jansen J. Prediction of success from a multidisciplinary treatment program for chronic low back pain. Spine 1997; 22:990-1001. A prospective cohort study including 90 disabled CLBP patients admitted to an 8-week program of functional restoration and behavioral support using pre and post-treatment standardized tests. Independant predictors for return to work rate after 1 year were initial application for pension, poor expectation concerning back to work, job as truck driver and change in depression and disability rating during treatment. Main predictors for pain intensity were initial application for pension, time off work, trunk extension test and change in disability ratings. The study impressively demonstrated the predictive power of cognitive variables.

A retrospective study of a large sample (1084 patients with CLBP and herniated nucleus pulposus). In patients with only abnormal advanced imaging results or clinical findings, therapy outcome was better in those who were operated on rather than conservatively treated. In patients with an unconfirmed diagnosis, treatment was no more successful than no treatment.

    • Hoogen van den HJM, Koes BW, Devillé W, Eijk van JTM, Bouter LM. The prognosis of low back pain in general practice. Spine 1997; 22:1515-1521. Three hundred and eighty-nine patients with specific or nonspecific low back pain were prospectively investigated over 1 year following an initial visit in general practice. Several potential predictors known from the literature (medical and work-related variables, a physician judgment of psychosocial problems) could not be confirmed. Predictors of relapse were physical therapy and history of surgeries. One shortcoming of this study was a possible selection bias as a result of the 40% attrition rate.

The influence of psychological variables on postoperative anxiety and physical complaints in patients undergoing lumbar surgery. Pain 1997; 69:19-25. One hundered and twenty-six patients undergoing lumbar surgery were investigated pre-and postoperatively during hospitalization. Preoperative anxiety and leg pain predicted postoperative anxiety after controlling for influence of age, gender and medical variables. Preoperative anxiety and fatigue predicted idependantly postoperative physical complaints. Coping behavior (blunting versus monitoring) revealed no predictive power. All assessments were carried out using standardized, reliable instruments.

This is a prospective cohort treatment study with 50 severe disabled CLBP patients undergoing a multidisciplinary functional restoration program. Waddell's nonorganic signs test revealed no predictive power using the criteria return to work, re-injury rate, health utilization and subsequent surgery. One hundred and twenty-eight currently employed chronic pain patients undergoing a multidisciplinary treatment program were investigated before treatment and seven times at follow-up until 30 months after treatment. This study used an elaborated questionnaire assessing 68 possible job complaints regarding physical, mental and psychological stressors. At 1-month follow-up, 79% of the patients were correctly classified by the predictors 'intent to return to work', education and several physical and psychological job stressors. The 30-month follow-up was predicted by 'intent', job stress, age, gender and education.

    • Psychosocial factors in the workplace - Do they predict new episodes of low back pain?
    • Papageorgiou AC, Macfarlane GJ, Thomas E, Croft PR, Jayson MIV, Silman AJ. Psychosocial factors in the workplace - do they predict new episodes of low back pain? Spine 1997; 22:1137-1142. The same population as that enroled in the Macfarlane study [51]. Predictors of occurence of low back pain until 12-month follow-up were perceived inadequacy of income in men, conflicts with colleagues in women, dissatisfaction with work and social class. Patients with perceived inadequacy of income and of lower social class showed an enhanced consulting behavior.

    • Goal setting as a predictor of return to work in a population of chronic musculoskeletal pain patients
    • Tan V, Cheattle MD, Mackin S, Moberg PJ, Esterhai JL Jr. Goal setting as a predictor of return to work in a population of chronic musculoskeletal pain patients. Int J Neurosci 1997; 92:161-170. Fifty-nine chronic pain patients undergoing universitary pain management were prospectively studied pre and 18 months post. The pre-treatment goal to return to work was the best predictor for return to work at follow-up in addition to age, marital status, education and time off work. Gender and compensation status were not predictive.

    • Blake C, Garrett M. Impact of litigation on quality of life outcomes in patients with chronic low back pain. Ir J Med Sci 1997; 166:124-126. Twenty CLBP patients underwent a multidisciplinary pain management program including exercises and behavioral interventions. Patients with ongoing litigation (n=11) did not a show a quality of life improvement (SIP) despite significant improvements in physical fitness and pain.

    • The effect of compensation involvement on the reporting of pain and disability by patients referred for rehabilitation of chronic low back pain
    • Rainville J, Sobel JB, Hartigan C, Wright A. The effect of compensation involvement on the reporting of pain and disability by patients referred for rehabilitation of chronic low back pain. Spine 1997; 22:2016-2024. This ia a prospective cohort study of 192 CLBP patients undergoing a spine rehabilitation program. Ninety-six patients with compensation claims had enhanced pain, depression and disability at baseline. Among 85 patients who completed the program, compensation patients showed a similar length of treatment and similar physical fitness after treatment. Despite improved depression and disability at follow-up, they showed more pain than the noncompensation group.

    • Employment and physical work activities as predictors of future low back pain
    • Macfarlane GJ, Thomas E, Papageorgiou AC, Crogt PR, Jayson MJ, Silman AJ. Employment and physical work activities as predictors of future low back pain. Spine 1997; 22:1143-1149. A population-based longitudinal study in England. The study comprised 1412 people free of low back pain with a 12-month follow-up. The rate of LBP in the follow-up period was 35%. Main predictors were jobs that involved lifting/pulling/pushing objects of at least 25 lbs or prolonged periods of standing/walking. However, no relationship between years of exposure to these risk factors and magnitude of risk was observed.
