It shows for the first time, however, a derepression of the endothelin gene by cyclosporin. If this observation can be substantiated it is an important step in our understanding of cyclosporin nephrotoxicity.

This line of work by Hadley and his colleagues is interesting and possibly very important because it pinpoints a new mechanism of tubulitis and cell-mediated destruction of tubules. This mechanism is brought about by TGF-β, and it remains to be seen whether this effect is greater in cyclosporin-treated patients than in those on other agents.
