Low insulin secretion and high fasting insulin and C-peptide levels predict increased visceral adiposity. 5-year follow-up among initially nondiabetic Japanese-American men. Diabetes 1996; 45:1010-1015. The development of visceral adiposity in relation to fasting and stimulated insulin and C-peptide levels was investigated prospectively in a sample of Japanese Americans. It is suggested that greater insulin resistance and reduced insulin secretion may precede visceral fat accumulation.

Atherosclerosis 1994; 107:239-246. Levels of visceral fat were higher in patients with CAD compared with healthy controls matched for sex, age and body mass index. These cross-sectional findings provided further evidence that visceral fat accumulation may play an important role in the occurrence of CAD, regardless of obesity.

    • Relationships of abdominal obesity and hyperinsulinemia to angiographically assessed coronary artery disease in men with known mutations in the LDL receptor gene
    • Gaudet D, Vohl MC, Perron P, Tremblay G, Gagné C, Lesiège D, et al. Relationships of abdominal obesity and hyperinsulinemia to angiographically assessed coronary artery disease in men with known mutations in the LDL receptor gene. Circulation 1998; 97:871-877. The most substantial increase in the risk of angiographically determined CAD patients with familial hypercholesterolemia was observed among abdominally obese and insulin-resistant individuals. This study provides support for the notion that the hyperinsulinemic-insulin-resistant state of abdominal obesity is a powerful predictor of CAD, even in a population at very high risk, such as patients with familial hypercholesterolemia.

It is reported that changes in the waist circumference, as opposed to changes in the waist/hip ratio, represent a better anthropometric predictor of changes in visceral adipose tissue accumulation that occur with age.

    • A single threshold value of waist girth identifies normal-weight and overweight subjects with excess visceral adipose tissue
    • Lemieux S, Prud'Homme D, Bouchard C, Tremblay A, Després JP. A single threshold value of waist girth identifies normal-weight and overweight subjects with excess visceral adipose tissue. Am J Clin Nutr 1996; 64:685-693. This study provides reference values of waist circumference corresponding to critical levels of visceral adipose tissue above which metabolic complications are most likely to occur. Although the relationship between the waist circumference and visceral adipose tissue accumulation is age-specific, it does not appear to be affected either by sex or by the degree of obesity.
