Am J Resp Crit Care Med 1997, 156:1760-1764. Using the protocol that has previously been applied to a middle school in New Mexico (see Sporik et al. [4]), the authors surveyed 1054 children and identified 135 with symptoms of asthma. Detailed study of these children identified 48 who had bronchial reactivity. The study confirmed the significance of sensitization to mite, cat, or cockroach, or any combination, in an area where these allergens were commonly present in houses. Using multiple regression analysis, it was found that total serum IgE, sensitization to pollens, passive smoke, and social class did not contribute to the risk of asthma or symptoms.

The ISAAC survey used written and video questionnaires to study 463,801 children aged 13 to 14 years in 155 centers in 56 countries. The investigators found wide variation, both between different countries and within countries. In the United Kingdom, for example, asthma prevalence ranged from 20% to 37%, depending on the center. Wide variation in asthma prevalence in countries with multiple study centers speaks to the difficulty of collecting such data and to probable regional variations. Unfortunately, in countries with a single study center, that center was used to represent data for the entire country, which may not reflect country-wide prevalence.

Brit Med J 1997. 315:717-721. The study compared two large cohorts of children who were born in 1958 and 1970 and responded to a questionnaire at age 16. The prevalence of eczema increased from 3.1% to 6.4% between the two studies. The prevalence of hay fever also increased from 1 2.0% to 23.3%. The prevalence of both disorders was influenced by birth order, maternal age, duration of breast feeding, maternal smoking, and social class, However, none of these could explain the observed increases. Also, in this study, the allergic diseases were more common in higher social classes, yet it is well documented that the asthma epidemic in the United States has had greatest impact on poor children.

The investigators surveyed 4470 children, ages 6 to 15 years, for respiratory illnesses such as bronchitis, hay fever, and asthma as well as for a variety of symptoms such as chronic cough, wheezing, sneezing, and conjunctivitis. Consistent with earlier studies, the researchers did find an association of air pollution with bronchitis, but they found none with wheezing, asthma, and hay fever.

The investigators surveyed 470 children in a polluted city and 611 children in a cleaner city. Children from the polluted city reported significantly more upper respiratory symptoms and significantly more physician-diagnosed asthma. Curiously, reports of wheezing were not significantly different between the two areas, suggesting that wheezing children in the polluted areas were more likely to be labeled asthmatic.

N Eng J Med 1997, 336:1356-1363. The National Cooperative Inner City Asthma Study (NICAS) studied a large cohort of children with asthma from poor families. Information was available on skin tests, home allergen concentrations, and several measures of asthma severity. Although there were considerable problems with the skin testing and no serum data, the results indicated that sensitization to cockroach, mite, and Alternaria were common in this population. The strongest association with hospitalization for asthma was the combination of sensitization to cockroach and the presence of allergen in the child's bedroom. The study confirms the significance of cockroach allergens in the inner city. However, the study provides no evidence that there has been an increase in cockroach allergen exposure.

The authors recruited 62 pairs of monozygotic and 62 pairs of dizygotic twins with discordant histories of asthma. Among the monozygotic twins, skin sensitivity to the indoor allergens dust mite, cat, and cockroach was significantly associated with asthma, whereas sensitivity to Alternaria, Aspergillus, and grass pollen was not. By contrast, among dizygotic twins, sensitization to both groups of allergens was associated with asthma. The results suggest that sensitization to outdoor allergen is an epiphenomenon related to genetic difference, while sensitization and exposure to indoor allergens is likely to be a direct environmental cause of asthma.

This study was in part a follow-up of the work of Herxheimer and Schaefer from 1974, which found asthma to be uncommon among traditional Inuit villagers. Asthma was still found to be uncommon, although there was a significant prevalence of decreased airway function apparently related to passive smoke, particularly exposure in utero: 80% of the mothers studied reported smoking during pregnancy. In addition, 32% of the children studied smoked. Within this population there was very little relationship between atopy and decreased airway function.

This study surveyed 9844 residents of the city of Jimma, Ethiopia, and 3032 residents of nearby rural villages for asthma, wheezing, and cough. In addition, they skin-tested one quarter of the participants with Dermatophoides pteronyssinus and mixed threshings. As expected, asthma and wheezing were both more common in the city (asthma 3.6%, wheezing 1.2%). Surprisingly, sensitization to D. pteronyssinus was actually higher in rural areas (11.8%) than in the city (4.0%). Yet none of the 119 mite-allergic individuals in the rural areas reported wheezing or asthma.

Using National Health and Nutrition Survey data to provide normal values, the authors evaluated a cohort of children in New York City for body mass index. They compared the asthmatic children to other children in New York and to the country in general. The results show that in a city where obesity is known to be a problem among children, the asthmatic children are heavier than the others. This report provides the first clear evidence of an epidemiologic association of obesity with asthma and provides strong evidence that there has been a change in the phenotype of asthmatic people.
