This study used euglycemic and hyperglycemic clamp techniques to demonstrate impaired glucose disposal, and a failure to increase glucose oxidation and suppress lipid oxidation in response to insulin infusion in obese versus non-obese Caucasian adolescents. This study also provided direct evidence of a relationship between visceral fat and insulin secretion and insulin sensitivity in obese, but not in non-obese female adolescents.

Visceral fat was positively correlated with triglycerides and negatively with HDL cholesterol.

The only published study to date examining the metabolic correlates of visceral fat in a sample comprised entirely of pre-pubertal children (Tanner stage I). Although both visceral and subcutaneous abdominal fat, as assessed by computed tomography scanning, were related to basal and postchallenge insulin in simple correlation analysis, only subcutaneous abdominal fat was independently and positively related to insulin area (among African-American children) after adjusting for total fat; neither index of abdominal fat was independently and positively related to basal and 30 min insulin. These results indicate that, before puberty, total and abdominal subcutaneous fat are more closely related to insulin production and action than is visceral fat.

In this study of 101 African American and Caucasian obese and non-obese pre-pubertal children, visceral fat (by computed tomography scanning) was highly variable and not related to percent body fat, though highly correlated with absolute fat mass and subcutaneous abdominal fat. Visceral fat relative to subcutaneous abdominal fat, was lower in African American than in Caucasian children but was similar in boys and girls.

This study examined the relationships between abdominal fat (by MRI) and metabolic risk factors in normal-weight Caucasian and African-American girls matched for chronological age, bone age, body weight, body mass index. Tanner stage (1-2), and socioeconomic status. Visceral fat was not associated with insulin or lipid parameters in either group, whereas subcutaneous abdominal fat was associated with both basal and 2 h insulin among African-American, but not Caucasian, girls. These results indicate an ethnic difference in the relationship between abdominal fat and metabolic risk, and suggest that subcutaneous, rather than visceral, abdominal fat is associated with basal and postchallenge insulin in young females.
