Transplantation 1997; 64:1261-1273. This report describes a meta-analysis of the entire literature on QoL in organ transplantation published through 1996. Six types of transplantation in adults were considered: kidney, pancreas/combined kidney-pancreas, heart, lung/ combined heart-lung, liver, and bone marrow. The review considers whether the weight of evidence from 218 independent investigations indicated that QoL, examined prospectively, improved from pretransplant to post-transplant; that transplant recipient QoL was better than that of patient comparison groups; and that recipient QoL equalled that of healthy nonpatient samples.

This report examines predictors of QoL and psychiatric distress outcomes after lung transplantation in 60 recipients and various groups of lung candidates. A variety of cross-sectional and longitudinal analyses indicated that transplant recipients showed statistically reliable mental health and QoL advantages relative to their own pretransplant status and relative to other transplant candidate groups. Multivariate analyses indicated that, after controlling for physical health and demographic parameters, pretransplant anxiety and psychopathology predicted post-transplant mental health; pretransplant anxiety predicted post-transplant QoL; and subjective sleep disturbances mediated the relationship between pretransplant anxiety and the post-transplant outcomes. This report is the only published study that has attempted to explain the mechanism by which pretransplant psychiatric status comes to affect post-transplant psychologic adjustment.

Chest 1998; 113:358-364. A sample of 24 patients were evaluated before lung transplant and again at multiple points during the first 2 years after transplant. Results indicated that both mental health and general QoL dramatically improved from pretransplant to post-transplant. Given the dearth of information regarding the temporal profile of either QoL or emotional status during the months or years after transplant, the study is noteworthy for data showing that these outcomes remained stable during the entire post-transplant follow-up period.
