    • Joshi GP, Ignagaki Y, White PF, Taylor-Kennedy LI, Wat L, Gervurtz C, et al. Use of the laryngeal mask as an alternative to the tracheal tube during ambulatory anesthesia. Anesth Analg 1997; 85:573-577. This multicenter study compares the use of the laryngeal mask versus the tracheal tube in 381 outpatients requiring general anesthesia. It shows that with the use of the laryngeal mask, significantly less early and late potoperative side effects occur, namely sore throat and nausea. In other words increased quality of anesthesia can be obtained with the use of the laryngeal mask, and this becomes extremely important in office-based anesthesia.

This recent study demonstrates the safety and effectiveness in maintaining and establishing an airway in spontaneous breathing anesthetized adults with two airway devices. Both of them very suitable for office-based anesthesia. The study also shows the differences between both airway devices, in terms of management and complications.

This fast-tracking concept becomes even more important in the office, than in any other ambulatory setting. In this study, they compared the recovery profiles of the new volatile anesthetics (sevo- and desflurane) and the IV anesthetic agent (propofol) as maintenance agents. Interestingly they found, that compared with propofol, sevo- and desflurane resulted in a higher percentage of outpatients being judged eligible for fast-tracking.

A rapid onset of action and short duration nondepolarizing muscle-relaxant is needed in the office. In this recent study they found that the more predictable recovery of mivacurium compared to rocuronium, makes it more suitable for short procedures.
