This article reports that keratoconjunctivitis sicca worsens as aqueous tear production decreases and is greatest in patients with Sjögren's syndrome, an autoimmune condition where the ability to reflex tear in response to sensory stimulation is lost. Compared with other tear film disorders, eyes with keratoconjunctivitis were found to have an irregular corneal surface by the xeroscope.

The authors report that there is an inverse correlation between the number of mucus producing conjunctival goblet cells and expression of a high molecular weight differentiation-dependent antigen, MEM2 (mucosal epithelial mucin 2), with the intensity of ocular surface rose bengal staining. This confirms the hypothesis of Feenstra and Tseng (Arch Ophthalmol 1992, 110:984-993.) that a normal mucus coating prevents rose bengal staining of the ocular surface epithelium.

Reports that the conjunctival epithelium in patients with Sjögren's syndrome keratoconjunctivitis sicca shows aberrant expression of the immune adhesion molecules, HLA class II, and ICAM-1 antigens. Furthermore, these cells contain increased levels of inflammatory cytokines. This paper provides new evidence that there may be an immunological mechanism for keratoconjunctivitis sicca.

These findings are consistent with previous reports that androgen hormones decrease lymphocytic infiltration in the lacrimal glands of these mice and also increase release of secretory component from cultured lacrimal gland acinar epithelium.
