For the first time, in vivo confocal microscopy was used to quantitatively assess the thickness of anterior corneal fibrosis during wound healing. After lamellar keratectomy in the rabbit, significantly less fibrosis was measured when corneas were treated with blocking antibodies to TGF-β.

Confocal microscopy was used to assess epithelial thickness, surface epithelial cell area, corneal thickness, depth of necrosis, inflammation, fibrosis, and endothelial injury following surfactant-induced ocular irritation in rats and rabbits. This is the first study to demonstrate the ability of confocal microscopy to characterize the pathobiology of ocular irritation in vivo.

Curr Eye Res 1997, 16:214-221. The in vivo quantitative CMTF technique is presented and used to estimate epithelial and corneal thickness in both rabbits and humans. CMTF measurements of corneal thickness were in agreement with ultrasonic pachymetry measurements, and both epithelial and corneal thickness were obtained with excellent reproducibility. This study establishes in vivo CMTF as an important new technique for measuring corneal sublayer thickness.

Acanthamoeba keratitis was diagnosed using confocal microscopy and confirmed by epithelial biopsy. Evidence of concurrent herpes simplex virus (HSV) infection was identified in nine patients, suggesting that Acanthamoeba may be present as a secondary or opportunistic infection in patients infected with HSV. Screening of patients with HSV infection for Acanthamoeba using confocal microscopy or other means is therefore advisable.

Confocal microscopy was performed in a patient with Fusarium solanae keratitis. Distinctive filamentous structures were detected intrastromally. Similar images were obtained from a culture of the patient's corneal scraping, confirming the in vivo observation. This study demonstrates the ability of confocal microscopy to diagnose fungal keratitis and monitor treatment.

Confocal microscopy of 16 patients with infectious crystalline keratitis revealed distinct, needle-like opacities in some patients and amorphous intrastromal deposits in others. These confocal findings resembled the histopathology reports; however, the confocal resolution was not adequate to determine bacterial morphology. Furthermore, no correlation was found between confocal appearance and the responsible etiologic agent. This study demonstrates that not all infectious agents can be distinguished using confocal microscopy, and physical biopsy of the cornea is still often necessary to confirm diagnosis.

Eight patients were examined by confocal microscopy from 7 days to 34 months after photorefractive keratectomy (PRK). Regenerating subepithelial nerves were observed in the area of photoablation as early as 7 days after PRK; however, nerves remained somewhat abnormal at all time points examined. Hypercellularity and stromal scarring could also be detected up to 30 months after PRK. This study demonstrates the unique capabilities of confocal microscopy for following corneal wound healing over time.

This study established CMTF as an important new tool for quantitative assessment of initial photoablation depth, temporal changes in epithelial, stromal and corneal thickness, and unbiased haze estimation after PRK. As expected, the change in corneal thickness measured by CMTF was highly correlated with the change in spherical equivalent refraction. Furthermore, a significant correlation was found between CMTF haze estimates and clinical haze grading using slitlamp examination.

Images of the tear film were obtained in human patients using a noncontact objective lens, and assessed based on the degree of debris in the tear film, variability of the interference pattern over time, linearity of the interference pattern, number of dry spots seen, and relative thickness of the tear film. Changes in these parameters were significantly correlated with physiologic variables in normal individuals and disease states, suggesting that confocal microscopy may represent a new paradigm for characterizing tear film abnormalities.

Eight patients were evaluated by confocal microscopy 2 to 16 weeks following laser in situ keratomileusis. Increased reflectivity was detected at the anterior stromal interface. Nerve regeneration and changes in epithelial morphology were also evaluated.
