Clin Rheumatol 1997, 16:129-132. Forty patients with spondyloarthropathies were studied, of whom only four patients (10%) had clinical symptoms that correlated with radiological enthesitis. Thus, the authors question the clinical significance of enthesitis, traditionally regarded as a pathologic hallmark of spondyloarthropathy.

J Rheumatol 1997, 24:912-915. Amyloid deposits detected through ASFA were found in 10 of 137 patients with AS, of whom only five patients developed dinical amyloidosis. Consequently, the detection of amyloid deposits by this method does not invariably infer poor prognosis.

Br J Rheumatol 1997, 36:677-682. Seventy percent of 26 patients with AS had pulmonal abnormalities when examined by high resolution CT. Interestingly, four patients had ILD, a disease complication not previously noted in AS. Plain radiographs failed to detect such abnormalities.

Two of the patients had undergone surgery, and after 2 years another patient was subjected to surgical treatment. Clearly, such cervical abnormalities often develop in AS, but rather infrequently lead to neurological complications.

In spite of a rather high frequency of HLA B27 (6%), AS was not found among 215 first-degree relatives of 48 B27-positive Fula twin pairs. Moreover, distribution of B27 subtypes could not explain the rarity of the disease. The authors suggested the presence of some non-B27 protective factor reducing the prevalence of AS.

After a mean disease duration of 16.5 years, approximately half of one hundred patients with adult-onset primary AS were working full-time. Twenty-four patients with AS who had 43 Charnley low-friction hip arthroplasties were evaluated after a mean of 22.7 years. The overwhelming majority was free of pain, and none had clinically important heterotopic ossification. The results are promising, and contrary to earlier reports on hip surgery in AS, postoperative soft tissue ossification appear to represent only a minor problem.
