This paper emphasizes that the melanoma "epidemic" is 1) worldwide, among persons of Caucasion skin type; 2) not attributable to increased sensitivity of diagnosis or change in criteria; and 3) accompanied by a rise in mortality due to melanoma. In men, the prevalence melanoma is increasing faster than that of any other cancer; in women, it is second only to the rising risk of lung cancer. Non-dermatologists need to improve their diagnostic skills with pigmented lesions.

    • Berg P, Lindelof B: Differences in malignant melanoma between children and adolescents. Arch Dermatol 1997, 133:295-297. Using data from the compulsory Swedish Cancer registry, the authors found that the incidence of melanoma among adolescents (14-20 years) had doubled over the past 10 years, while the incidence in children less than 14 years old was unchanged. This study underscores the need for all physicians, including pediatricians, to become aware of the warning signs of melanoma.

J Am Acad Dermatol 1996, 35:529-538. This review examines 289 patients with large melanocytic nevi, about two fifths deriving from a prospective registry of giant nevi and about three fifths from a literature review. Of these 289, 33 had central nervous system melanosis. Patients with posterior axial lesions have an increased risk for neurocutaneous melanosis, especially if associated with multiple satellite nevi. Magnetic resonance evaluation of the central nervous system should be considered in these patients.

Thirty-four (12%) patients had primary cutaneous melanoma; in 2 patients the melanoma developed at extracutaneous sites. Other neural crest and mesenchymal tumors also occurred in 13 patients. The incidence of malignancy in axial nevi seems to be greater than in extremity nevi, although the latter are much less common and this observation may not hold. Importantly, melanoma was never observed arising from satellite nevi.

Dermabrasion of congenital nevi, particularly when undertaken in the neonatal period, is efficacious in reducing pigmentation in most patients. Unfortunately, melanomas have been reported to arise from residual nevus following partial thickness excisions, hence whether partial excision methods offer more than cosmetic benefit remains to be determined.

Congenital nevi in 18 prepubertal children were treated with the Q-switched ruby laser. Pigmentation was reduced by an average of 50%, but partial repigmentation usually occurred within 6 months. With further improvements in the technology, the primary application of laser therapy may be to enhance the appearance of low-risk lesions, such as satellite nevi in children with giant congenital nevi. Because the technique cannot provide tissue for histopathology, it will always be inappropriate when there is any suspicion of malignancy.

This paper demonstrates that using predetermined criteria for intraepidermal melanocytic atypia, dermatopathologists can reproducibly grade degrees of melanocytic dysplasia in biopsies.

This case-controlled study enrolled sufficient patients to allow the investigators to separately assess the risk of melanoma associated with having atypical nevi vs. having numerous common nevi. Although earlier studies had suggested that these were separate risk factors, they tend to occur in the same patients (ie, patients with atypical moles tend to have lots of moles) and it was difficult to evaluate their separate contributions to melanoma risk. In this study, atypical (ie, clinically dysplastic nevi) were most strongly associated with melanoma risk; one atypical nevus conferred a two-fold increased relative risk, and 10 or more conferred a 12-fold increased risk; whereas the magnitude of risk associated with numerous common nevi was two-to four-fold. Interestingly, congenital nevus-like nevi in this study were not associated with melanoma risk.

    • Greene MH: Genetics of cutaneous melanoma and nevi. Mayo Clin Proc 1997, 72:467-474. The controversies surrounding the genetics of melanoma and dysplastic nevi are reviewed. There are probably multiple loci for dominantly inherited familial melanoma. There is evidence that dysplastic nevi (outside of familial melanoma kindreds) are also dominantly inherited, although whether the same loci that determine melanoma are involved is not known. The author also reviews the literature that demonstrates that an abnormal mole pattern with dysplastic or atypical nevi is a strong clinical predictor of melanoma risk.
