    • Endarterectomy for asymptomatic carotid artery stenosis
    • Executive Committee for the Asymptomatic Carotid Atherosclerosis Study: Endarterectomy for asymptomatic carotid artery stenosis. JAMA 1995, 273:1421-1428. The Asymptomatic Carotid Atherosclerosis Study (ACAS) trial is a prospective multicenter randomized trial of carotid endarterectomy in patients with asymptomatic carotid stenosis (greater than 60% reduction in lumen diameter) that was conducted at 39 clinical centers in the United States and Canada from 1987 to 1994. In this study, 1662 eligible patients, among 42,000 patients screened (4%), were randomized: 828 to the surgical group (carotid endarterectomy plus risk factor modification plus 325 mg aspirin per day), and 834 to the medical group (risk factor modification and 325 mg aspirin per day). The primary outcome events were stroke or death in the perioperative period, and stroke during follow-up. Transient ischemic attack [TIA] was not an outcome event; therefore, the results do not mention the number of patients who eventually experienced episodes of transient monocular visual loss during follow-up.) After a median follow-up of 2.7 years, cumulative risk (estimated risk) over 5 years of any perioperative stroke or death and of ipsilateral stroke during follow-up was estimated to be 11% for medical group patients and 5.1% for surgical group patients, a 5.9% absolute risk reduction, and a 53% relative risk reduction in the surgical group over 5 years. The authors concluded that "the incidence of cerebral infarction can be reduced by carotid endarterectomy." This apparently straightforward result has provoked considerable controversy, however.

This article is important because it is the first large prospective study aimed to assess prevalence and associations of asymptomatic retinal emboli in a defined older population. Asymptomatic retinal emboli were found in 1.4% of 3654 asymptomatic Australian persons aged 49 years or older. Eighty percent of the emboli were cholesterol type. This study provides accurate prevalence rates for asymptomatic retinal emboli in the elderly and confirms associations with hypertension, smoking, and vascular disease.

This article reviews all the studies showing that atherosclerotic disease of the aortic arch is an independent risk factor for ischemic stroke and a strong marker for recurrent vascular events. This location of atherosclerotic disease mainly involves patients older than 60 years. Its diagnosis is easy with transesophageal echocardiography, which is accurate, safe, and well-tolerated for the examination of the aortic arch, even in elderly patients. Treatment remains undetermined, but controlled therapeutic trials are about to start.

This substudy of the North American Symptomatic Carotid Endarterectomy Trial (NASCET) provides important information about the prognosis of patients presenting with transient monocular visual loss related to high-grade internal carotid artery stenosis. The authors identified 129 medically treated patients with 70%-99% stenosis of the internal carotid artery who had their first transient ischemic attack as the entry event into the trial. Fifty-nine patients with transient monocular visual loss (TMVL) were compared with 70 patients with hemispheric transient ischemic attack (TIA). On aspirin alone, the risk of cerebral infarction at 2 years was 16.6% ± 5.6% for the patients with TMVL and 43.5% ± 6.7% for patients with TIAs. Risks increased with severity of stenosis for both.

In this study of 45 consecutive patients with hemodynamically significant carotid occlusive disease, the authors were able to demonstrate a good correlation between ischemic oculopathy and retrograde flow in the ophthalmic artery. They also suggested that the occurrence of retrograde ophthalmic artery flow may be the main determinant with regards to development of chronic ocular ischemic changes.

Information on carotid artery disease was available in 63 patients, and of these, 10 patients had plaques only, 20 had stenosis of the internal carotid artery (percent stenosis not detailed), and 33 were normal. However, the group with carotid evaluation was not a random sample and is, therefore, likely to overestimate somewhat the prevalence of carotid artery disease in nonarteritic anterior ischemic optic neuropathy.

The two patients described in this report had embolic cilioretinal artery occlusion and atheromatous disease of their ipsilateral carotid artery, and support the debated theory of "embolic anterior ischemic optic neuropathy" (AION). Because the short posterior ciliary arteries also perfuse the optic disc, the author suggests that emboli from the carotid artery can also occasionally produce AION by occluding the blood supply to the optic disc.

This is a retrospective study of 100 asymptomatic patients who had undergone MR imaging with sequences that could be used to evaluate the relation between the intracranial optic nerve and the supraclinoid internal carotid artery. Seventy percent of patients had contact of one or both optic nerves with the carotid artery, 12% had bilateral compression of the optic nerve while only 5% had unilateral compression. The risk of compression was directly proportional to the diameter of the carotid artery.

Cranial nerve palsy was present in 23 of 190 consecutive patients, among whom five had oculomotor palsy (two third nerve palsy, two sixth nerve palsy, one fourth nerve palsy). Associated symptoms included headache in all five patients, Horner's in two, and sixth nerve involvement in one. Mechanisms of these cranial nerve palsies from carotid dissection are discussed.
