This study documents multiple problems in conforming with the standards of care advocated by the NAEPP. In a population of innercity patients with moderate to severe asthma, management was characterized by underutilization of anti-inflammatory therapy and overutilization of β-agonists, inability to use inhalation devices properly, inadequate communication between patient and physician, lack of an action plan to be utilized in the event of an acute exacerbation, and inadequate physician intervention during the acute stages of the exacerbation.

The authors reviewed 252 articles reporting the results of interventions designed to help patients adhere to therapy. Only 13 met the following minimum criteria: randomized controlled trial, one or more measure of adherence with prescribed medication, one or more measure of treatment outcome, and at least 80% follow-up. Seven studies were associated with improvement in adherence, and six led to improvement in treatment outcome. However, even the most effective interventions did not lead to substantial improvement.

The author writes, "According to Skrabanek, humane medicine itself is being threatened, not just by erroneous scientific ideas about etiology and therapy, but by 'coercive healthism'." He warns that physicians may be transformed from their traditional role of personal concern and individual care to new activities that involve uninvited intrusion and zealous advocacy and new roles as bureaucratic custodians of public health.
