The authors report their results in 180 patients who were referred to them for palliative treatment of recurrent malignant effusion. Sixty-three of these patients had a "trapped lung syndrome" and had a pleuroperitoneal shunt inserted, whereas thoracoscopic talc pleurodesis was done on the others. Effective palliation was achieved in more than 95% of the patients in each group.

J Thorac Cardiovasc Surg 1996, 112:385-391. This article describes the technique and results of video-assisted thoracoscopy in a consecutive series of 100 episodes of spontaneous pneumothorax in 97 patients. A 3% rate of recurrence of pneumothorax was observed, and complications were seen in 10% of the cases (no deaths occurred after surgery). Conversion to thoracotomy had to be done in five patients.

This is a report on 1365 VATS procedures performed to treat pneumothorax in 19 German institutions. The overall rate of recurrence was 6.5%; no recurrences were seen when talc pleurodesis was used, and a high rate of recurrence (16.4%) was seen after attempts at fibrin-glue pleurodesis. When no pleurodesis procedure was added to thoracoscopic bleb resection, the overall recurrence rate was 10.2%.

Report on the effectiveness of and rate of complications associated with thoracoscopic talc poudrage done on 88 patients with malignant mesothelioma and 272 patients with metastatic pleural effusion. The rate of successful pleurodesis was 90.2% at 1 month and 82.1% for the lifelong follow-up. Adverse effects included one death, which occurred 3 days after the procedure in a patient with end-stage disease; fever in 9.8% of cases; empyema in 2.5% of cases; pulmonary infection in 0.8% of cases; and neoplastic invasion of the thoracoscopic scar in one patient. The authors conclude that thoracoscopic talc pleurodesis should be done early in the management of patients with malignant effusion to prevent respiratory failure associated with the poor condition of the patient at the time the procedure is done.

Results of gross and microscopic examinations are detailed, and the authors conclude that effective pleurodesis can be obtained with either talc slurry or thoracoscopic talc insufflation in this model.

From a randomized study that included 57 patients with malignant pleural effusion, these authors conclude that video-assisted thoracoscopic talc insufflation was not significantly superior to talc slurry for the outcome of pleurodesis.
