This is a noncomparative study on the utility of videothoracoscopy in the management of pleural pathologic processes in 306 patients, and in 23 of the patients it was for hemothorax. The causes of the hemothorax were post-traumatic, after open heart surgery and iatrogenic and malignant causes. The hemothorax resolved in all of the 23 patients. The chest tube was removed a mean of 2.8 days postoperatively and hospital stay averaged 4.3 days.

Chest 1996, 109:1514-1519. This open, prospective, multicenter trial from five hospitals in Mexico tested the efficacy of intrapleural fibrinolytic therapy in patients with complicated hemothorax and empyema. The overall success rate was 92% as assessed by improvement on radiologic and pulmonary function tests.

A retrospective analysis based on data published in the English medical literature of 110 patients with thoracic endometriosis syndrome (TES). Pleuropulmonary manifestations of TES include pneumothorax, hemotysis and lung nodules in descending frequency. Hemothorax was more often associated with pleural and pelvic endometriosis compared with other manifestations.

Van Veldhuizen PJ, Taylor S: Chylothorax: a complication of a left subclavian vein thrombosis. Am J Clin Oncol 1996, 19:99-101. This complication was reported in a patient who presented with malignant lymphoma following treatment for Hodgkin's lymphoma. A left subclavian vein thrombosis and right chylous effusion developed. The Chylothorax was postulated to result from obstruction of the orifice of the thoracic duct drainage, causing back pressure and leakage of chyle from pleural lymphatics into the pleural space. The chylous effusion gradually resolved with anticoagulation treatment.

This is the fourth patient with sarcoidosis reported in the literature who presented with chlyothorax. The patient had multiple enlarged lymph nodes in the upper mediastinum, preazygos, precarinal, and para-aortic area. The mechanism of chylothroax formation in this particular patient was uncertain.

This article describes the occurrence of a right chylothorax in an 82-year-old patient in whom Mycobacterium tuberculosis was isolated from the pleural fluid and was the only manifestation of intrathoracic tuberculosis. After 9 months of treatment with antituberculosis drugs, the chylothorax resolved.

This is a retrospective analysis of eight patients with isolated thoracic duct injury due to neck trauma. The site of the injury was within the Poirier's triangle bordered by the arch of the aorta, the left subclavian artery, and the vertebral column. Initial conservative management with chest tube drainage in five patients was unsuccessful. All patients required surgical intervention. The authors advocated early operative management.

This article is a retrospective study of the management of chylothroax developed after pulmonary resection for lung carcinoma. Seven (1.8%) out of the 384 patients reviewed had chylothorax. The diagnosis of chylothorax was made on the first or second postoperative day. Conservative treatment with chest tube drainage and parenteral hyperalimination alone was unsuccessful. The authors recommended rapid reoperation for postoperative chylothorax when the patient can withstand the procedure. A "1-week trial" of conservative treatment is indicated only in patients whose postoperative condition is poor.

Sieczka EM, Harvey JC: Early thoracic duct ligation for postoperative chylothorax. J Surg Oncol 1996, 61:56-60. This article describes four patients who had postoperative chylothorax after thoracotomy for resection of bronchogenic carcinoma (n = 3) and chondorsarcoma of the rib (n = 1). All three of the patients who underwent early reoperation with ligation of the thoracic duct after unsuccessful conservative treatment survived. The patient who had delayed thoracic duct ligation (after 30 days of conservative treatment) died. On the basis of this experience, the authors recommend early surgery when conservative treatment (7 to 14 days) fails.
