This report of the first three Swedish cases of pneumoconiosis in dental technicians independent of silica exposure. One developed significant respiratory impairment, while the other two were diagnosed early and left the workplace without disablility. All impairment occurred in dental laboratories where poor local exhaust ventilation existed.

Based on death certificate data, there appears to be a decreasing prevalence of silicosis and coal workers pneumoconiosis in the U.S. This decline in mortality correlates with legislation which was begun in the 1970s mandating changes in ambient air monitoring in the workplace and modification of the environment. In the coal industry, these changes have been implemented despite increasing productivity per miner.

Appl Occup Environ Hyg 1996, 11:937-941. Inhalation of freshly fractured silica by rats resulted in a greater inflammatory pulmonary response when compared to aged silica. Also, electron spin resonance spectroscopy verified increased surface free radical activity of freshly fractured silica. The clinical significance is not clear but it may relate to exposures in workers such as sandblasters and rock drillers, occupations with high exposure to freshly fractured silica, and the increased incidence of silicosis in this group.

These two mortality studies (references 23 and 24•) include a population of workers in the dusty trades, and not just workers selected because they were recognized to have silicosis and received disability. As such, they are perhaps more influential in defining the relationship between silica and lung cancer.

This case report of two miners with International Labour Organization profusion category 2 pneumoconiosis and normal lung function describes recovery of dust, proinflammatory cytokines, and inflammatory cells by whole lung lavage. Long-term follow-up of pulmonary status in such patients will be required to assess the role of dust burden reduction in disease course modification.
