Am J Respir Crit Care Med 1996, 153:1850-1856. This is one of the few studies that has attempted to determine the bioactivity and proinflammatory action of cytokines in BALF in patients with ARDS. A biologic assay was developed to quantify the proinflammatory activity by measuring the level expression of the intracellular adhesion molecule on a culture sample of human epithelial lung adenocarcinoma A549 cell line after incubation with BALF. The relative importance of IL-1 and TNF was also assessed using their respective inhibitors. The proinflammatory activity was compared between normal volunteers, patients with lung masses, patients with ARDS at day 3, patients in their first week after the onset of ARDS, and patients at risk for the development of ARDS. The majority of the proinflammatory activity in BALF was believed to be due to IL-1 and not TNF. This finding emphasized the need to quantify bioactivity of the various cytokines that have been documented to be elevated. Second, proinflammatory activity was maximal on day 3 after the onset of ARDS. Finally, patients at risk for the development of ARDS had high proinflammatory activity in their BALF.

This study evaluated the relationship between cytokines and cellularity in BAL fluid of patients with ARDS. Eighty-two patients underwent BAL testing on day 3 and weekly thereafter. Neutrophil chemokines (IL-8 and epithelial cell-derived neutrophil activator-78) monocyte chemokines (MCP-1 and MIP-1), and IL-1β and its inhibitor IL-1RA were measured. Using statistic methods, a fair correlation between concentrations of IL-8 and epithelial cell-derived neutrophil activator-78 and neutrophil levels in BAL fluid was found. BAL fluid was tested in 5 patients and 2 control subjects for PMN chemotactic activity. All ARDS patients had significant chemotactic activity, which was reduced significantly with the addition of an antibody to IL-8.

This study evaluated the level of neutrophil activation by measuring the expression of L-selectin and CD11b in patients with ARDS. the Level of F-actin was also evaluated. In comparison to control subjects (n = 15), patients with ARDS (n = 14) had lower levels of L-selectin and F-actin content and higher levels of CD11b, reflecting a higher degree of neutrophil activation. The level of activation correlated statistically with the degree of lung injury and to the levels of TNF, IL-6, and IL-B.

Early in the course of patients who progressed to ARDS, 92 kD gelatinase was found to be elevated in BALF, and tissue inhibitor of metalloproteinase-1 was found to be elevated in both BALF and plasma in patients with ARDS when compared to controls. Moreover, in patients with prolonged ARDS, the the ratio of 92 kD gelatinase to tissue inhibitor of metalloproteinase remained elevated. Neither the 92 kD gelatinase levels nor tissue inhibitor of metalloproteinase levels in BALF, plasma, or their ratio correlated with clinical indices of the severity of lung injury. However, nonsurvivors had higher levels of 92 kD gelatinase and tissue inhibitor of metalloproteinase as compared with survivors. The elevation of 92 kD gelatinase may reflect the turnover of extracellular matrix proteins in lung injury, and the increased levels of tissue inhibitor of metalloproteinase may interfere with the reparative processes involved in lung remodelling after ARDS.

Ann Intern Med 1995, 122:17-23. This important study evaluated a large number of patients (117) at several time points during their ICU stay. It confirmed an earlier study with fewer patients demonstrating elevated levels of type III procollagen peptide in patients with ARDS. Type III procollagen peptide was found to be a reasonable prognostic indicator for mortality, although significant overlap existed between groups. Its main importance stems from the fact that it provides a stimulus for future studies into the pathogenesis of the fibroproliferative response in ARDS.

Ann Intern Med 1996, 125:191-196. This purpose of this seminal study was to determine the importance of the balance of pro- and anti-inflammatory cytokines in determining mortality in ARDS. In a prospective cohort study, the intra-alveolar concentrations of the proinflammatory cytokines (TNF, IL-1β, and IL-8) and the anti-inflammatory cytokines (IL-10 and IL-1 receptor antagonist) in patients with early ARDS were measured. Concentrations of proinflammatory and anti-inflammatory cytokines within the alveolar air spaces were significantly elevated in patients with ARDS compared with control subjects. Most importantly, a high significant correlation was found between low concentrations of anti-inflammatory cytokines and subsequent patient mortality rates. The authors postulate that failure to mount a localized intrapulmonary anti-inflammatory response early in the course of ARDS contributes to more severe organ injury and worse prognosis and suggest that augmentation of anti-inflammatory cytokine defenses might be a beneficial therapeutic approach to patients with ARDS.
