Comprehensive neurodevelopmental testing of five infants who received transplants is reported with a description of pretransplant course, potential damaging events, and use of sedation to maintain hemodynamic stability. Gross motor skills were the only area that was significantly lower than the general population (P < 0.001). Factors that might influence outcome were tested, with length of hospital stay and use of sedative medication associated with lower language and social skills.

Long-term outcome of CPB versus TCA in 155 children repaired for TGA as measured by the BSID showed a psychomotor development index with a 6.5 point deficit (P = 0.01) in the patients with TCA. There was an inverse relationship to the duration of arrest (P = 0.02). The mental development index (cognition) was not related to method of support. Seizure activity and PDI was associated (P=0.002) as were abnormal MRIs (P<0.001).

The effect of CPB versus TCA was evaluated in 114 children with mixed CHD diagnoses as compared with 54 children with atrial septal defect repair. The bypass group had shorter reaction times (by 2 to 3 seconds) but not difference in IQ. There was a relationship with duration of arrest to lower IQ.

Neurodevelopmental outcome of 104 children with reported CHD. Mean IQ was 90, with a range of 30 to 133. Cerebral palsy was present in 22% of this group. Deep hypothermia was associated with IQ < 85 (P < 0.001) and later cerebral palsy (P=0.02). Survivors of hypoplastic left heart syndrome had a median IQ of 66 with only one above 70 and 57% had cerebral palsy. The authors feel that a majority of patients do well but that there is a significant morbidity.
