J Natl Cancer Inst 1995, 87:1365-1371. This study confirms that type-specific HPV persistence, particularly with a high viral load, leads to persistence of CIN lesions rather than regression. This has important implications not only for the natural history of CIN but also for HPV testing.

The majority of attempts to demonstrate potential clinical use of HPV testing have employed the detection of viral DNA. This report describes an approach for the demonstration of transcriptionally active viral infection which therefore reflects viral activity rather than just viral presence. It remains to be determined whether this will be of more clinical value than DNA testing.

The association of monoclonality of CIN lesions with HPV type has important implications for understanding the biology of the involvement of HPV infection in cervical neoplasia. From a diagnostic point of view, this paper identifies two groups of low-grade lesions: those which are monoclonal and associated with high/intermediate-risk HPV types, and those which are polyclonal and associated with low-risk HPV types. Whether this provides any prognostic information additional to knowledge of HPV type remains to be determined.

Lancet 1996, 347:1523-1527. The author reports a study of the use of a recombinant vaccinia virus for immunization of a small group of patients with advanced cervical carcinoma. Three of eight patients developed HPV-specific antibodies and one developed a cytotoxic T-cell response. These findings are encouraging but larger-scale trials are clearly required.

Cancer 1996, 77:507-513. A study of clear cell adenocarcinoma showing an association with microsatellite instability rather than mutation of conventional oncogenes. This suggests that this recently described alternative mechanism of carcinogenesis, initially delineated in hereditary non-polyposis-associated colonic carcinoma, may be important in some gynaecological malignancies.
