This important experimental study in rats shows that blocking the endothelin-A receptor with a specific antagonist prevents myocardial remodeling (dilatation). myocardial fibrosis, and myocardial dysfunction. Thus. this study provides a compelling rationale for the use of endothelin receptor antagonists to prevent disease progression in heart failure.

This study was the first to show that both TNF-α mRNA and protein are expressed in the failing hearts of patients with ischemic heart disease and dilated cardiomyopathy, whereas there was no evidence for TNF-α mRNA and protein expression in the nonfailing heart. This study also showed that TNF receptors were downregulated in the failing heart as compared with normal hearts, suggesting that the heart was a target organ for TNF-α in heart failure. Thus, these studies provide an important conceptual understanding of an important source of cytokine production in heart failure.

This clinical study was the first to document the presence of elevated levels of soluble TNF receptors in patients with heart failure and the first study to show that increased levels of TNF receptors correlated with worse prognosis in patients with heart failure. This study also showed that TNF-α levels were most elevated in the patients with the worst clinical outcomes.

J Am Coll Cardiol 1996, 27:1201-1206 This important clinical study was the first to show that both TNF-α and interleukin-6 levels were elevated in direct relation to deteriorating NYHA functional class for patients enrolled in SOLVD. This study also showed that there was no relationship between neurohormonal levels and elevated cytokine levels, suggesting that the elaboration of neurohormones was not the mechanism for cytokine expression in heart failure.
