2. Bauer JJ, Harris MT, Grumbach NM, Gorfine SR: Laparoscopic-assisted intestinal resection for Crohn's disease. Dis Colon Rectum 1995, 38:712-715. This small series of laparoscopic resection for patients with Crohn's disease shows that it can be accomplished without significant complications. Fixed abdominal masses and fistulas are a risk factor for conversion.
3. Fleshman JW, Fry RD, Birnbaum EH, Kodner IJ: Laparoscopic-assisted and minilaparotomy approaches to colorectal diseases are similar in early outcome. Dis Colon Rectum 1996, 39:15-22. The benefits of LAC in terms of ileus and length of hospital stay remain superior to the results achieved with minilaparotomy; both have advantages over open procedures.
4. Ludwig KA, Milsom JW, Church JM, Fazio VW: Preliminary experience with laparoscopic intestinal surgery for Crohn's disease. Am J Surg 1996, 171:52-56. This sizable series showing that the laparoscopic approach is possible in patients with inflammatory bowel disease contains a useful definition of feasibility.
5. Lumley JW, Fielding GA, Rhodes M, Nathanson LK, Siu S, Stitz RW: Laparoscopic-assisted colorectal surgery: lessons learned from 240 consecutive patients. Dis Colon Rectum 1996, 39:155-159. This very large series of patients with multiple indications for surgery reported a low conversion rate and low incidence of complications. It is one of the few series with results broken down for individual surgical procedures.
6. Slim K, Pezet D, Riff Y, Clark E, Chipponi J: High morbidity rate after converted laparoscopic colorectal surgery. Br J Surg 1995, 82:1406-1408. To avoid complications, the authors encourage early conversion in cases that cannot easily be performed laparoscopically.
7. Senagore AJ, Luchtefeld MA, Mackeigan JM: What is the learning curve for laparoscopic colectomy? Am Surg 1995, 61:681-685. One of the most complete papers on the subject of the learning curve, this paper demonstrates improvements in operative times and rates of conversions and complications. It also demonstrates how a high conversion rate reduces the magnitude of the advantages seen with this procedure.
8. Lord SA, Larach SW, Ferrara A, Williamson PR, Lago CP, Lube MW: Laparoscopic resections for colorectal carcinoma: a three-year experience. Dis Colon Rectum 1996, 39:148-154. Laparoscopic colectomy fulfils basic oncologic parameters as defined by surgical margins and number of lymph nodes harvested. In addition, when analyzed by type of procedure, laparoscopic colectomy demonstrates consistent advantages as compared with open colectomy.
9. Ortega AE, Beart RW Jr, Steele GD, Winchester DP, Greene FL: Laparoscopic bowel surgery registry: preliminary results. Dis Colon Rectum 1995, 38:681-686. A large number of colorectal procedures are being performed laparoscopically, according to results from this national registry. Of note, 88% of surgeons participating noted no violation in oncologic principles during laparoscopic colectomy.
10. Wishner JD, Baker JW Jr, Hoffman GC, Hubbard GW II, Gould RJ, Wohlemuth SD: Laparoscopic-assisted colectomy: the learning curve. Surg Endosc 1995, 9:1179-1183. This paper demonstrates an impressive reduction in operative times with experience but possibly at the expense of improvements in conversion and complication rates.

Dis Colon Rectum 1996, 39:S53-S58. This analysis of the early pretrial experience of surgeons participating in the National Cancer Institute phase III intergroup laparoscopic colectomy trial demonstrates reassuring survival curves for patients with colon cancer who are treated laparoscopically.
12. Hoffman GC, Baker JW, Doxey JB, Hubbard GW, Ruffin WK, Wishner JA: Minimally invasive surgery for colorectal cancer: initial follow-up. Ann Surg 1996, 223:790-798. Presenting a relatively small number of patients with colorectal cancer, but with a substantial follow-up of at least 24 months, this paper is reassuring in demonstrating similar survival curves for laparoscopic colectomy as compared with open colectomy, with no reports of wound or trocar site recurrences.

An interesting analysis of the learning curve as demonstrated by operative times of individual surgeons during their initial experience with laparoscopic colectomy.

Bennett CL, Stryker SJ, Ferreira MR, Adams J., Beart RW Jr: The learning curve for laparoscopic colorectal surgery: preliminary results from a prospective analysis of 1,194 laparoscopic-assisted colectomies. Arch Surg 1996, in press. This paper attempts to determine the number of cases required to "learn" laparoscopic colectomy, as defined by a reduction in the incidence of complications. This is not a procedure that should be attempted by surgeons who have low-volume colorectal practice.

A relatively large multi-institutional series of laparoscopic resections for cancer, with early survival curves similar to those for open surgery.
18. Reissman P, Salky BA, Pfeifer J, Edye M, Jagelman DG, Wexner SD: Laparoscopic surgery in the management of inflammatory bowel disease. Am J Surg 1996, 171:47-51. From this large series of patients with inflammatory bowel disease, the authors are able to make specific recommendations regarding laparoscopic technique for this disease process.

A review of surgical oncologic principles forms the background to this description of the need for the phase III National Cancer Institute-supported intergroup trial that is currently enrolling patients.
