National and state estimates for 1990 and projections for the year 2020 of the prevalence of self-reported arthritis and other rheumatic diseases and related activity limitations are presented. The prevalence rate of self-reported arthritis and other rheumatic conditions in the United States is projected to increase from 15% (37.9 million) of the 1990 population to 18.2% (59.4 million) of the estimated 2020 population. Activity limitation attributed to these conditions is projected to increase from 2.8% of the 1990 population to 3.6% of the 2020 population. Prevalence rates were higher for older persons, women, residents of nonmetropolitan areas, and those with less education or lower income.

The economic, social, and psychologic impact of musculoskeletal conditions in the United States was reviewed and estimated. The cost of musculoskeletal conditions was $149.4 billion in 1992, of which 48% was due to direct medical care costs and the remainder was due to indirect costs resulting from wage losses. This amount translates to approximately 2.5% of the Gross National Product. Each year, persons with musculoskeletal conditions make 315 million visits to physicians, have more than 8 million hospital admissions, and experience approximately 1.5 billion days of restricted activity. These conditions are responsible for a sizable amount of health care use and disability, and they significantly influence the psychologic status of affected individuals as well as their families.

Health Psychol 1995, 14:399-408. The utility of measuring both positive and negative affective states for assessing RA patients was examined in three independent samples of men and women with RA. Confirmatory factor analyses of each sample indicated that positive and negative affect constituted separate, negatively correlated factors. The relationships among disease variables, coping, and affect were found to be consistent with a model in which coping mediates the relationship between disease variables and positive and negative affect.

Arthritis Care Res 1996, 9:279-291. The effects of a spouse-assisted pain-coping skills training intervention on pain, psychologic disability, physical disability, pain coping, and pain behavior were evaluated in 88 patients with knee osteoarthritis. At the completion of treatment, patients who had spouse-assisted coping skills training had significantly lower levels of pain, psychologic disability, and pain behavior, and higher scores on measures of coping attempts, marital adjustment, and self-efficacy.
