Ann Neurol 1996, 40:446-450. The authors have shown how the presence of volumetric atrophy in both hippocampal regions can influence prognosis. Patients with unilateral hippocampal atrophy were 90% seizure free whereas only 65% of patients with bilateral hippocampal atrophy were seizure free at 1 year after surgery.

The opportunity to study metabolic recovery with successful treatment is important. In this work the authors provided evidence for recovery of metabolic dysfunction at a distance from the epileptogenic focus once seizures abated in some patients. This study also suggested that MRS may be a useful technique in the investigation of metabolic recovery and prognosis.
