Intestinal immunoglobulin secretion was studied in patients with AIDS-related enteric infection, patients with AIDS without enteric infection, and control patients. Intestinal IgA and IgM plasma cells were quantitated using digital image analysis and total IgA as well as anticryptosporidium IgA, IgM, and IgG measured in serum and feces. Contrary to previous reports, mucosal IgA plasma cells, fecal IgA, and serum IgA were normal or increased in patients with AIDS as compared with control patients. It appears that local defects in cell-mediated rather than humoral immunity may predispose to HIV-associated eneteric infection.
