The Dana Consortium on therapy for HIV dementia and related cognitive disorders: Clinical confirmation of the American Academy of Neurology algorithm for HIV-1 associated cognitive/motor disorder. Neurology 1996, 47:1247-125. A report by the Dana consortium on therapy for HIV dementia and related cognitive disorders confirmed the American Academy of Neurology's algorithm and found it rare to have both cognitive and functional impairments without associated neurologic and behavioral deficiencies. Dementia was found to be an independent predictor of physical outcome in affected patients.

The inflammatory response system of brain: Implication for therapy of Alzheimer and other neurodegenerative diseases
-
McGeer PL, McGeer EG: The inflammatory response system of brain: Implication for therapy of Alzheimer and other neurodegenerative diseases. Brain Res Review 1995, 21:195-218. The authors review recent progress in research involving the inflammatory responses of brain. The suggest that immune dysregulations in the CNS may be autotoxic for neurons, exacerbating the fundamental pathology in Alzheimer disease and perhaps other degenerative neurological disorders. New agents directed against the brain inflammatory processes showed in studies of Alzheimer disease may have broad therapeutic applications.

Study of receptor-mediated neurotoxins released by HIV-1 infected mononuclear phagocytes found in human brain
-
Giulian D, Yu J, Xia L, Tom D, Li J, Lin SN, Schwarz R, Noonan, C: Study of receptor-mediated neurotoxins released by HIV-1 infected mononuclear phagocytes found in human brain. J Neurosci 1996, 16:3139-3153. In vitro reconstitution studies indicates that the macrophage product, NTox, produced following HIV-1 infection activate NMDA subtypes of glutamate receptor-operated synaptic ion channels at picomolar concentrations. This factor presumably results in necrotic neuronal injury/death.

Immunologie NO synthase: Elevation in severe AIDS dementia and Induction by HIV-1 gp41
-
Adamson DC, Wildemann B, Sasaki M, Glass JD, McArthur JC, Christov VI, Dawson TM, Dawson VL: Immunologie NO synthase: elevation in severe AIDS dementia and Induction by HIV-1 gp41. Science 1996, 274:1917-1921. The levels of inducible nitric oxide synthase in severe ADC coincided with increased expression of the HIV-1 coat protein gp41 in affected brain tissue. Inducible nitric oxide synthase was induced by gp41 in primary cultures of mixed rat neuronal and glial cells. The data suggest that gp41-induced nitric oxide formation may contribute to the severe cognitive dysfunction associated with HIV-1 infection.

Encephalitic brains from SIV-infected animals have increased levels of the C-C chemokines, macrophage inflammatory protein-1α and -β, RANTES monocyte chemoactic protein-3, and the C-X-C chemokine interferon-inducible protein-10. These findings suggest that one or all of these chemokines could be involved in monocyte recruitment into the CNS.

J Immunol 1997, 158:3499-3510. An artifical BBB model was constructed that allows an analysis of HIV-1 and monocyte penetration into the brain. The transendothelial migration of monocytes through this BBB system demonstrates that immune activation, not viral infection, is the major event that permits immune cell entry into brain tissue.

Am J Pathol 1996, 149:1027-1053. This model of HIV-1 encephalitis utilizes SCID mice lacking functional B and T-cells due to a defect in T-cell receptor and immunoglobulin variable chain rearrangement. This allows the animals to accept human monocytes without rejection. The model demonstrates that human monocytes can synthesize neurotoxins that cross species (human to mouse) and can reproduce key aspects of HIV encephalitis in the animals.
