Subgroup analysis showed an improved local-regional control rate in T3-4 and also in larynx carcinoma in favor of the accelerated arm.

This study demonstrated the importance of cumulative time factors in determining locoregional control achievable with surgery plus radiation in patients with a risk of relapse. There was also a trend in favor of higher locoregional control rate with accelerated fractionation.

Preliminary results (in French) of a large EORTC (No22851) randomized trial that included more than 500 HNSCC patients treated either with accelerated radiotherapy (72 Gy in 5.5 weeks) or with conventional (70 Gy in 7 weeks), The analysis showed an improvement in the local-regional control, disease-free survival (LCR, DFS) (P < 0.01) and also in overall survival of borderline significance (P = 0.07) in favor of the accelerated arm. Increased late effect was also evident, suggesting that an interval between fraction of 4 hours might have been too short, given the relatively high dose per fraction used ( 1.6 Gy × 3 times daily) to allow optimal DNA repair in normal tissues.

Int J Radiat Oncol Biol Phys 1995, 32:162. An interim report of a randomized trial which included a limited number of patients and compared hyperfractionated accelerated radiation therapy with conventional radiation therapy, and showed a benefit in favor of the accelerated arm.

Proceedings of the 4th International Conference on Head and Neck Cancer 1996:53. A randomized trial comparing 66 Gy in 6.5 weeks to 66 Gy in 3.5 weeks that was stopped after inclusion of 82 patients because of increased grade IV late toxicity in the accelerated arm. No difference was seen in tumor control.

Munro AJ: An overview of randomised controlled trials of adjuvant chemotherapy in head and neck cancer. Br J Cancer 1995, 71:83-91. Meta-analysis based on 54 randomized trials comparing locoregional treatment alone versus the same treatment combined with chemotherapy. The analysis suggested no benefit for the use of neoadjuvant chemotherapy; however. it showed a significant improvement in survival in favor of the use of concomitant radiochemotherapy.
