The authors review 1252 cases of scleral buckle procedures, analyzing the cause for primary failure. They also analyze the outcome after repeat surgery with a segmental or encircling buckle. They contend that after failed buckle surgery, the second procedure need not be gas injection or a vitrectomy. The majority of cases can be successfully managed with a segmentai explant.

Using data collected from the Silicone Oil Study, the authors demonstrate that joint knowledge of the location of PVR and the tightness of funnel for a retina with 9 to 12 clock hours involved by fixed folds has prognostic utility for eyes with anterior and posterior PVR.

Eyes with anterior PVR and clinically significant posterior PVR had a better visual prognosis if silicone oil was used.

The authors demonstrate that intraocular tissue plasminogen activator can be safely used to lyse postoperative fibrin occluding the inferior peripheral iridectomy in these eyes.
