Using three sensitive laboratory methods, the authors detected platelet-reactive antibodies in 30.8% of transfused patients with hematologic diseases. This rate of alloimmunization compares with 20% to 70% reported by others.

Using citric acid, the authors decreased HLA expression on human platelets while preserving platelet-specific glycoproteins. Three of four patients transfused with HLA-eluted platelets had satisfactory outcomes. A fourth patient developed chills after the transfusion.

This editorial includes a report of 52 crossmatch-compatible platelet transfusions in 19 refractory patients, with satisfactory post-transfusion corrected platelet count increments in 65%. A trial of acid-treated platelets resulted in 20% to 30% platelet loss. Tests to predict viability showed decreased but acceptable results.
