Fourteen patients with Barrett's esophagus and 12 control patients underwent endoscopy with methylene blue staining of the esophagus. The overall accuracy of methylene blue staining for intestinal metaplasia was 95%.

Am J Gastroenterol 1996, 91:981-982. Two hundred thirty-seven patients underwent elective upper endoscopy for clinical indications. Short-segment Barrett's esophagus was suspected in 42 patients and was confirmed by biopsy in 48%. Only 53% of these individuals presented with typical or complicated GERD.

Am J Gastroenterol 1996, 91:1507-1511. One hundred seventy patients without Barrett's esophagus were evaluated for intestinal metaplasia of the esophagogastric junction. Although 16 patients had intestinal metplasia, in 12 of these the esophagogastric junction and squamo-columnar junction were at the same level. Pyrosis and regurgitation were more common in those with intestinal metaplasia.

Six of 166 patients with Barrett's esophagus who underwent surveillance endoscopy developed esophageal cancer. These patients were compared with a group of similar patients who had not undergone surveillance endoscopy. The former group was found to have their cancer identified at a significantly earlier stage.

Only two of these patients were determined to have died from their esophageal cancer.

J Thorac Cardiovasc Surg 1996, 111:1135-1140. One hundred thirteen patients with Barrett's esophagus underwent antireflux procedures. Morbidity related to the surgical procedure occurred in 36%. Of the 72 who had postoperative endoscopy, all had persisting Barrett's esophagus. Three patients developed esophageal adenocarcinoma for an incidence of one in 274 patient-years of follow-up.

Thirty-six patients with Barrett's esophagus underwent endoscopy with laser-induced fluorescence spectroscopy to detect high-grade dysplasia. Ninety-six percent of nondysplastic mucosa was classified as benign, and 90% of high-grade dysplasia samples were identified as premalignant.

Gastrointest Endosc 1996, 44:700-705. Partial regression of the length of Barrett's esophagus is reported with use of omeprazole 60 mg/d. Esophageal acid exposure was normalized in 12 of 14 patients. The antisecretory effect of omeprazole as measured by 24-hour gastric pH monitoring was maintained throughout the year of therapy.

High-dose omeprazole (mean dose, 56 mg/d) to control esophageal pH and endoscopic multipolar electro-coagulation reversed a segment of Barrett's esophagus in 10 patients. The entire Barrett's segment was reversed in five patients at the time of this report. Only 7% of the ambulatory treatment sessions resulted in a complication, one requiring hospitalization.

Thirty-six patients with Barrett's esophagus with dysplasia or superficial carcinoma were treated with photodynamic therapy and omeprazole. At the time of this report 10 patients had reversal of Barrett's esophagus and another 19 had Barrett's without dysplasia. Twenty-one patients developed an esophageal stricture.

Thirty consecutive patients with Barrett's esophagus with high-grade dysplasia underwent esophagectomy, with 13 having adenocarcinoma in the resected specimen. Eight patients had stage I cancer with seven being alive at a mean follow-up of 18 months. One death and three major complications occurred postoperatively.
