An interesting account of the diagnosis of urethral sphincter failure with a review of current and past literature on diagnosis and surgical management. They concluded that intrinsic sphincter deficiency was best recognized by history and clinical examination in conjunction with documentation of severe stress leakage, a fixed urethra and a low Valsalva leak point pressure. They also concluded that the pubo-vaginal sling was still the most widely accepted corrective surgery.

22. Carr LK, Herschorn S, Leonhardt C: Magnetic resonance imaging after intraurethral collagen injected for stress urinary incontinence. J Urol 1996, 155:1253-1255. This procedure describes using MRI to look at the periurethral tissues after collagen bulking injections for urethral sphincter failure. Thirty-two women were scanned a median of 12 months after injection. Good visualization of the injected collagen was achieved, which matched the volume injected, but did not correlate with the final clinical outcome.
