This randomized trial suggests that adjunctive subconjunctival mitomycin C therapy does not further improve the final intraocular pressure that results from primary trabeculectomy combined with phacoemulsification and intraocular lens implantation in patients with primary open-angle glaucoma.
12. Glaucoma Laser Trial Research Group: The glaucoma laser trial (GLT) •• and glaucoma laser trial follow-up study 7. Results. Am J Ophthalmol 1995, 120:718-731. This paper reports on the long-term follow-up of a major multicenter clinical trial that compared laser trabeculoplasty with medicine as initial treatment for primary open-angle glaucoma. The study concluded that initial treatment with argon-laser trabeculoplasty was at least as effective as initial treatment with topical medication over a median 7-year follow-up period.

This surgical outcome study showed that progressive glaucomatous damage occurs in about one third of eyes with moderate to severe primary open-angle glaucoma within 6 years after trabeculectomy.

This retrospective review concludes that laser suture lysis is safe and effective in augmenting aqueous filtration after trabeculectomy with mitomycin C. A longer time interval between surgery and laser suture lysis may result in both a lesser reduction in intraocular pressure and a lower incidence of subsequent hypotony.

This retrospective review suggests that the Molteno implant drainage device offers a reasonable long-term outcome in eyes with intractable glaucoma. However, there was an ongoing failure rate similar to that seen after other filtration surgery.
17. The Fluorouracil Filtering Surgery Study Group: Five-year follow-up of the •• fluorouracil filtering surgery study. Am J Ophthalmol 1996, 121:349-366. The long-term follow-up of this multicenter clinical trial suggested the use of subconjunctival 5-fluorouracil after trabeculectomy after previous cataract surgery or unsuccessful filtering surgery, but it cautioned against its routine use in patients with good prognoses.

This is a good review of existing and planned clinical trials in glaucoma. The authors find it particularly exciting that intraocular pressure is not an end point in these new trials and that consideration is being given to the previously ignored but extremely important issue of overall patient well being.
