Examines outcome of 449 HLA-A, -B, and -DR serologically matched unrelated donor transplants, comparing those matched (by molecular typing) for HLA-DRB1 and HLA-DQB1 with those mismatched at HLA-DRB1 and/or -DQB1. Documents increased risks of grade III-IV acute graft-versus-host disease with either DRB1 or DQB1 mismatching.

Ann Intern Med 1997, 126:184-192. Reports on 798 transplant recipients surviving at least 5 years after bone marrow transplantation. Subsequent mortality was 8% at 10 years post-transplantation. Most patients had normal or minimally reduced performance scores and had resumed full-time work or school. Major causes of late death were recurrent cancer, secondary cancers, and chronic graft-versus-host-disease.
