    • Randomised controlled trial of vitamin E in patients with coronary disease: Cambridge Heart Oxidation Study (CHAOS)

Lancet 1996, 347:781-786. The effect of vitamin E (400 or 800 IU daily) was studied in 2002 patients with CHD. Treatment reduced the primary trial endpoint of cardiovascular death and non-fatal myocardial infarction from 64 in the placebo group to 41 in the vitamin E group (P=0.005). A nonsignificant excess of cardiovascular deaths was observed in the vitamin E group. This is the first prospective clinical trial with results consistent with the lipid oxidation hypothesis of CHD.

Probucol given to patients in the PQRST reduced serum vitamin E concentrations by 14%, secondary to cholesterol and triglyceride lowering, and reduced carotenoids by 30-40% because of reductions in lipoprotein particle size and competition for incorporation into VLDL. The use of a lipid-soluble antioxidant and cholesterol lowering drug might have unfavourable effects on blood levels of diet-derived antioxidants.

Beta-carotene was administered to 11036 male physicians at 50 mg on alternate days for 12 years; the effect on cancer and cardiovascular disease was compared in a randomized double blind study with 11035 men receiving placebo. No early or late differences were found in the overall incidence or mortality of malignant neoplasms or cardiovascular disease.

More than 18000 individuals at risk for lung cancer were treated with a supplement of 30mg beta-carotene and 25000 IU of retinol for four years; the relative risk of death from cardiovascular disease after the trial was 1.26. The combined treatment might have adverse effect on the risk of death from lung cancer and cardiovascular disease.

Supplementation decreased the release of reactive oxygen species, lipid oxidation, interleukin-1β secretion and monocyte-endothelial cell adhesion. The study provided evidence for an intracellular antiatherogenetic effect of α-tocopherol in monocytes.
