Clin Exp Rheumatol 1995, 13:11-16. The association between major histocompatibility complex class I, II, and III antigens and systemic sclerosis was measured in 41 Mexican mestizo patients with scleroderma, 113 of their first-degree relatives, and 85 ethnically matched controls. HLA-DR5 and HLA-DRw52 frequencies were higher in scleroderma patients compared with the matched controls (relative risk, 3.31, and 95% Cl, 1.3 to 8.3; versus relative risk, 2.4, and 95% Cl, 1.0 to 5.7). Ten of the 41 patients (24.3%) had DRB1*1104 subtypes compared with six of 85 healthy controls (7.0%) (relative risk, 4.25).

Normal controls consisted of 181 healthy individuals. A significant association was observed in the ACA-positive patients with DQB1*0501 (relative risk, 2.6; P = 0.001), and a significant negative association was observed with DQB1*0201 (relative risk, 0.33; P = 0.01). These HLA associations persisted after stratification of the ACA-positive patients.

Allele typing for HLA-DBP1 was done in 54 patients with idiopathic systemic sclerosis, 26 uranium miners with systemic sclerosis (either anti-Scl-70 positive or negative), and 70 unrelated healthy controls. Frequencies of DPB1*1301, *0601, and *0701 alleles were enhanced in anti-Scl-8-positive systemic sclerosis cases compared with healthy controls. A strong association between anti-Scl-70 positivity in systemic sclerosis patients and the glutamic acid residue of DPB1 was thus observed in comparison both with anti-Scl-70-negative systemic sclerosis patients (P = 0.0009) and with controls (P = 0.0007).

Cases of scleroderma (n = 21) were taken from computerized admission files from all the local hospitals (1976 to 1991). Each case was matched with two age- and gender-matched controls, discharged at the same time. Subject occupational histories were ascertained through structured questionnaires. A significant association was found (odds ratio, 9.28; 95% Cl, 1.08 to 243.8) in three men and one woman with scleroderma who had been exposed to organic solvents.

Data were analyzed from a 14-year follow-up (1976 to 1990) of the Nurse's Health Study cohort. Data were collected before the media began reporting on possible health risks from implants. Information on 87,501 women was collected from postal questionnaires and blinded medical record reviews; 1183 of those surveyed had received an implant. There were 516 subjects with definite connective tissue disease, of whom 14 had scleroderma. None of these 14 women had had an implant.

Analysis was performed on the records of 4229 female patients from a US rheumatologic practice (1986 to 1992). Of 721 patients (mean age, 43.8 years) with a connective tissue disease, 64 had scleroderma. One hundred fifty women had had breast implantation (mean age, 47.2 years). The study found no evidence that women with breast implants are at an increased risk for scleroderma or other connective tissue diseases. No scleroderma cases had been diagnosed among women with implants.

JAMA 1996, 275:616-621. A retrospective cohort study (1962 to 1991) of 395,543 female US health professionals (90.7% white and non-Hispanic; mean age, 51.7 years) who completed postal questionnaires. The study reported that 10,830 women had had breast implants and that 11,805 had connective tissue disease. Ten women with implants and 314 without implants had scleroderma (relative risk, 1.84; 95% Cl, 0.98 to 3.46). Five women with systemic sclerosis had had implants for less than 4 years (relative risk, 2.68; 95% Cl, 1.11 to 6.51). No clear trends could be shown with increasing interval since implantation. The study concluded that these data exclude the possibility of a high risk of connective tissue disease in women who have had breast implants.
