This report describes 13 zidovudine-treated patients who developed fatigue, myalgia, and diminished exercise tolerance but had normal muscle strength. Seven had moderate to severe mitochondrial abnormalities. The remaining six had only type II fiber atrophy on light microscopy but had mitochondrial abnormalities on electron microscopy. The observations describe the evolution of zidovudine myopathy and underscore the importance of mitochondrial abnormalities in its pathogenesis.

Arthritis Rheum 1995, 38:690-698. Eight Jamaican patients with polymyositis were studied; seven were found to be seropositive for HTLV-I. HTLV-I DNA was detected in DNA prepared from muscle biopsy specimens. By immunohistochemistry HTLV-I was shown to be mainly in mononuclear cells, but some staining of muscle fibers was seen.

In addition, they also had neurologic findings, lacked dysphagia, and had a poor response to treatment.

A case report of a patient with hepatitis C virus infection and polymyositis. Muscle tissue was subjected to PCR analysis to detect hepatitis C RNA. Only positive strands, the genomic RNA, and not negative strands, the replicative intermediates, could be detected, suggesting that viral replication does not occur in muscle.

As is found in most series, S. aureus was the most common cause. Risk factors included intravenous drug use, steroid use, and diabetes. CT and MRI criteria for diagnosing pyomyositis were developed and tested for their ability to identify pyomyositis.

Brook I: Pyomyositis in children, caused by anaerobic bacteria. J Pediatr Surg 1996, 31:394-396. Six children seen over a period of 16 years with anaerobic pyomyositis are described. All children had polymicrobial infections, some with mixed aerobic and anaerobic infections. Although the number of patients with typical aerobic pyomyositis seen during this period is not described, the report calls attention to the need to culture for anaerobic organisms when considering a diagnosis of pyomyositis.
