N Engl J Med 1995, 333:913-917. The authors performed a prospective cohort study of patients with acute low back pain treated by urban and rural primary care physicians, urban and rural chiropractors, orthopedic surgeons, and primary care providers at an HMO. Functional outcomes were similar for all groups, but primary care practitioners provided the least expensive care.

Shekelle PG, Markovich M, Louie R: Comparing the costs between provider types of episodes of back pain care. Spine 1995, 20:221-227. Using the RAND Health Insurance Experiment data the authors compared the costs of back pain episode by type of health care provider. Chiropractors had the highest number of patient visits per episode of back pain. Orthopedic surgeons had the highest cost per visit; however, because orthopedic surgeons required fewer visits of patients, outpatient costs between chiropractors and orthopedists were similar.

Outcomes for toenail procedures were prospectively compared for patients treated by podiatrists, general surgeons, orthopedic surgeons, and general practitioners within the National Health Service in Suffolk, England. Patient satisfaction was lowest for general surgeons and seemed to be closely linked to rates of problematic regrowth.

Weale AE, Bannister GC: Who should see orthopaedic outpatients: physiotherapists or surgeons? Ann Royal Coll Surg Engl 1995, 77(2 suppl):71-73. This study examined the possibility of having physiotherapists assume the role of consultant for ambulatory orthopedic referral patients. Each referral letter was initially screened by an orthopedic surgeon who decided whether the patients were likely candidates for surgery. All patients improved over time; however, physiotherapists appeared to have more success than orthopedic surgeons in treating patients with low back pain.
