This report from a cohort study demonstrates that left ventricular geometry has a major effect in prognosis in chronic uraemia. In patients with concentric LVH the left ventricular mass was associated with subsequent death, whereas in patients with left ventricular dilation the prognostic indicator was not the extent of LVH but the degree of left ventricular dilation.

This cohort study concludes that ischaemic heart disease occurs frequently in dialysis patients, that its adverse impact is mediated through the development of heart failure and that the most important potentially reversible risk factors are hypertension, hypoalbuminaemia and underlying cardiomyopathy.
9. Parfrey PS, Foley RN, Harnett JD, Kent GM, Murray DC, Barre PE: The outcome and risk factors for left ventricular disorders in chronic ureamia. Nephrol Dial Transplant 1996 (in press). This cohort study concludes that manifestations of cardiomyopathy are frequent in dialysis patients, and are associated with high risks of heart failure and death. Potentially reversible risk factors include anaemia, hypertension, hypoalbuminaemia, and ischaemic heart disease.

One hundred pateints with suspected CAD each had exercise, dipyridamole and dobutamine echocardiography, the results of which were compared with coronary arteriography. It was concluded that dobutamine echocardiography was a pivotal diagnostic tool when exercise was not feasible.
