Lancet 1996, 348:701-707. This paper describes the first trial of SPf66 in Asia. This is the most detailed evaluation ever conducted with a malaria vaccine and concludes that SPf66 is not protective against P. falciparum.

Trans R Soc Trop Med Hyg 1995, 89:418-421 The rectal route was compared with the intramuscular route. The efficacy was satisfactory although the bioavailability of the drug was lower than with the intramuscular injection. This route of administration could prove useful in situations where basic healthcare is lacking, and to reduce the risk of cross infections through contaminated needles.

Trans R Soc Trop Med Hyg 1995, 89: 528-530. This trial was conducted in 1991-1992 in Cameroon and the Congo. Amodiaquine had a cure rate by day 7 of 80% compared with 60% for chloroquine. Fifty per cent of the strains were resistant in vitro to chloroquine and 9% were resistant to amodiaquine. The investigators strongly suggest that amodiaquine should be used as a first-line treatment of uncomplicated malaria in chloroquine-resistant areas of Africa.

J Antimicrob Chemother 1995, 36:723-728 This open randomized trial compares high dose chloroquine (alone or with clindamycine) with Fansidar in Gabon. Failure rate was 32% in the chloroquine group but the rate of high grade failure was reduced to 7%. The chloroquine plus clindamycine achieved 94% cure rate and SP 98%.

This meta-analysis of mefloquine treatment in children under 5 years of age shows that the major problem in giving mefloquine to young children is early vomiting. A trial of artemether or quinine in children with cerebral malaria
-
•], this is the largest trial on severe malaria ever conducted. There was no difference in mortality or residual neurological sequelae between the two groups. Time to coma recovery was longer in the artemether group (26 h versus 20 h; P = 0.046). Parasite clearance time was shorter in the artemether group: 48 h versus 60 h. More children in the artemether group had convulsions: odds ratio = 1.86 (1.28-2.71).

N Engl J Med 1996, 335:76-83. A randomized double-blind trial in 560 adults with severe artemether malaria. There was no difference in mortality, but a more rapid paracite clearance time in the artemether group: 72 versus 90 h, a slower fever clearance time: 127 versus 90 h and a longer time to coma resolution: 66 versus 48 h, and a longer hospitalization: 288 versus 240 h. More patients in the quinine groups developed hypoglycaemia.

This is the first published study of pyronardine outside Chine. In this open trial, chloroquine had a cure rate of 44% and pyronardine 100% at 14 days. The authors suggest that pyronardine could replace chloroquine in Africa.

This is an interesting analysis, supported by epidemiological data of the relationship between transmission, morbidity and mortality from malaria. It suggests that certain interventions may not, in the long term, reduce the overall burden of the disease.
