J Clin Endocrinol Metabol 1995, 80:573-579. Interesting study that reports some of the first autopsy data on the immunochemistry of brain tissue in persons with PWS. The paper is weakened by incomplete proof of the presence of PWS in study patients as well as by a poor control group. Nevertheless, conclusions point in an interesting and perhaps an important direction for future research.

It also describes the imprinting center, a novel genetic element that controls the initial resetting of the parental imprint in the germline for all imprinted gene expression over a 1.5-2.5 Mb region within chromosome 15q11-q13.

They propose that 15q11-q13 contains a single imprinting center rather than separate paternal and maternal imprinting control elements.

The data presented here strongly support a model in which the paternal chromosome in PWS can carry an ancestral maternal epigenotype and the maternal chromosome of persons with Angelman syndrome may carry an ancestral paternal epigenotype.

A report of two cases of PWS with UPD in which a meiosis I nondisjunction error involving an altered chromosome 15 produced by both a translocation event and recombination was detected. The authors suggest that the structural rearrangements of the chromosome disrupted normal segregation of an imprinted region, resulting in UPD and PWS.

They report that this Snrpn gene is expressed in a wide range of somatic tissues and at high, approximately equal levels in all regions of the brain. Their findings are consistent with a key role for DNA methylation in the imprinted inheritance and subsequent gene expression of the human Snrpn gene.

This is the only controlled study to date that has evaluated the effects of growth hormone therapy in prepubertal children with PWS. Growth hormone test results, including IGF-1 levels, are reported for both children with PWS and obese control subjects. The positive effects of growth hormone on growth velocity, IGF-1 levels, and body mass index are reported. Similar studies of longer duration are still necessary to provide information about long-term benefits and improvement in final adult height in this population of children with PWS.
