Neurosurgery 1996, 38:265-271. This is an initial report on the use of preradiation chemotherapy done with the eight-drug-in-1-day approach. It shows the benefit of aggressive resection in children with medulloblastoma. It also reviews the early data on the efficacy of preradiation chemotherapy and the effect of various other risk factors on outcome in patients with disease.

This is a difficult-to-read but important study showing that survival rates in children with nondisseminated medulloblastoma were similar for children who received full-dose craniospinal radiation and those who received reduced-dose craniospinal radiation. This report also suggests that delaying radiation in children who do not have disseminated disease at time of diagnosis may result in poorer overall outcome.

This very interesting report describes the use of higher-dose chemotherapy plus autologous bone marrow rescue in children who failed to respond to initial chemotherapy for malignant high-grade glioma. The authors show impressive rates of disease control in patients who had local recurrence after high-dose chemotherapy supplemented by local radiotherapy. In contrast, independent of the use of high-dose craniospinal radiation therapy, patients who had craniospinal disease at the time of progression all failed to respond to treatment.
