Short-course treatment of visceral leishmaniasis with liposomal amphotericin B (AmBisome)

Clin Infect Dis 1996, 22:938-943. This paper reports a multicentre, dose-ranging clinical trial including 88 immunocompetent patients (56 children). Three groups were treated with total AmBisome doses of 24, 18 and 15 mg/kg, respectively, the follow-up being 1 year or longer. The 18 mg/kg regimen, which consisted of single doses of 3 mg/kg per day on days 1-5 and day 10, was found to be highly effective (98%), well tolerated and had a low hospitalization cost.

This is the first report on antimony-associated pancreatic necrosis, documented in an AIDS patient on the second day of treatment with pentavalent antimony. The patient had previously been treated with liposomal and conventional amphotericin B, and had suffered repeated relapses. This report should discourage the use of antimonials as maintenance therapy.

Two AIDS patients who relapsed after, or were intolerant to antimonial treatment, received 400 mg/day fluconazole plus 300 mg/day allopurinol for 3 weeks. This association was also established for maintenance therapy by reducing the fluconazole dosage to 200 mg/day. No replases have occurred during 11 months follow-up. This drug regimen should be investigated further.

Trans R Soc Trop Med Hyg 1995, 89:440. This is the first report on the efficacy of liposomal amphotericin B in dermal leishmaniasis. This finding suggests that the drug is not only sequestered in major macrophage-rich tissues, as previously thought, but can also reach dermal areas at therapeutic levels.

In this paper, a powerful immunomodulant polyribonucleotide was found to reduce by some 80% the Leishmania amastigote burden in animals treated for 10 days. Interestingly, the leishmanicidal effect appears to be caused by enhancement of amastigote killing through a nitric oxide-mediated mechanism.
