Findley L, Unverzagt M, Guchu R, Fabrizio M, Buckner J, Suratt P: Vigilance and automobile accidents in patients with sleep apnea or narcolepsy. Chest 1995, 108:619-624. This paper details the performance of patients with obstructive sleep apnea or narcolepsy on Steer Clear, a computer program simulating a long and monotonous highway drive. The authors concluded that patients with sleep apnea or narcolepsy performed more poorly on this test of vigilance than did control subjects and that impaired vigilance as measured by Steer Clear is associated with a high automobile accident rate in patients with either sleep apnea or narcolepsy.

Pack Al, Pack AM, Rodgman E, Cucchiara A, Dinges DF, Schwab CW: Characteristics of crashes attributed to the driver having fallen asleep. Accid Anal Prev 1995, 27:769-775. The authors investigated the characteristics of 4333 crashes attributed to the driver being asleep in the database at the Highway Safety Research Center at the University of North Carolina between 1990 and 1992. They found that crashes were primarily of the drive-off-the-road type (78% of the total) and took place at higher speeds (62% in excess of 50 mph). The fatality rate was of similar magnitude to that in alcohol-related crashes. Crashes occurred primarily at two times of day: during the nighttime period of increased sleepiness (midnight to 7:00 AM) and during the midafternoon "siesta" time of increased sleepiness (3:00 PM). Fifty-five percent of these crashes involved individuals 25 years of age or younger, with a peak age of occurrence at age 20 years.
